眼睑Merkel细胞癌手术并随访7年1例

Eyelid Merkel cell carcinoma operated and followed up for 7 years:a case report

Merkel细胞癌(Merkel cell carcinoma,MCC)是一种临床罕见的原发性皮肤神经内分泌癌,虽好发于头颈部,眼睑仍为少见。临床中误诊率高,恶性程度高,预后极差。目前MCC尚无成熟且有效的治疗方案。本文回顾性报道我科一例发生于眼睑的MCC患者,7年前行手术切除后联合放化疗治疗并存活至今。查阅复习相关文献并结合本病例表明,MCC手术切除为优选方案,术后辅以全身化疗及局部放疗对其转移及复发具有抑制作用。近年来免疫治疗及分子靶向药物的应用将为MCC的有效治疗翻开新的篇章。

Merkel cell carcinoma(Merkelcellcarcinoma,MCC)is a clinically rare primary cutaneous neuroendocrine carcinoma.Although it often occurs in the head and neck,the eyelid is still rare.In clinic,the misdiagnosis rate is high,the malignant degree is high,and the prognosis is very poor.At present,there is no mature and effective treatment for MCC.This paper retrospectively reports a case of MCC in our department who was treated with radiotherapy and chemotherapy after surgical resection 7 years ago and has survived ever since.Review of related literature and combined with this case show that surgical resection of MCC is the best option,combined with postoperative systemic chemotherapy and local radiotherapy can inhibit its metastasis and recurrence.In recent years,immunotherapy and the application of molecular targeted drugs will open a new chapter for the effective treatment of MCC.
