多方位呼吸功能改善干预在慢阻肺患者护理中的应用及对患者呼吸动力学指标的影响研究 被引量:6

摘要 目的:研究多方位呼吸功能改善干预的方法及对COPD患者呼吸功能、肺功能及生活质量的影响。方法:分组对照研究方案下将90例COPD患者分为两组,各组45例。YB组患者予以常规护理,YB+HX组在YB的基础上开展多方位呼吸功能改善干预。于护理前后,检测呼吸动力学指标(WOB、Raw、MVV)、肺功能指标(6MWD、FVC、FEV1),并采用CAT问卷、SGRQ问卷调查呼吸功能对患者日常健康、生活能力的影响。结论:护理后,YB+HX组患者WOB、Raw均低于YB组(P<0.05),MVV高于YB组(P<0.05),肺功能三项6MWD、FVC、FEV1均高于YB组(P<0.05)。同时,在CAT、SGRQ评分上,护理后YB+HX组患者均低于YB组(P<0.05)。结论:多方位呼吸功能改善护理从生理理化反应调节、功能锻炼、健康管理等各个方面促进COPD患者呼吸功能得到有效提高,可在改善呼吸动力学的同时提高患者肺功能,从而减轻呼吸功能障碍对患者日常健康、生活能力的影响,提高生活质量。 Objective:To study the promotion and improvement of respiratory capacity from multiple angles,and the influence on respiratory capacity,pulmonary tissue performance and quality of life in COPD patients.Methods:90COPD patients were divided into groups with 45 patients in each group.YB group patients were provided with general care,YB+HX group was set on the basis of YB to promote and improve the respiratory capacity from multiple angles.Before and after the implementation,respiratory dynamics indexes(WOB,Raw,MVV)and lung tissue physiological ability indexes(6MWD,FVC,FEV1)were detected,and CAT and SGRQ questionnaires were used to investigate the effects of respiratory function on patients’daily health and living ability.Results:After nursing,the improvement effects of WOB,Raw and MVV in YB+HX group were better than those in YB group(P<0.05),and the lung tissue physiological capacity of 6MWD,FVC and FEV1 were higher than those in YB group(P<0.05).Meanwhile,the CAT and SGRQ scores in YB+HX group were lower than those in YB group after nursing(P<0.05).Conclusion:Multi-directional nursing care for improvement of respiratory function from physiological and chemical response regulation,functional exercise,health management and other aspects to promote the respiratory function of COPD patients to be effectively improved,can improve the respiratory dynamics at the same time to improve the lung function of patients,so as to reduce the impact of respiratory dysfunction on patients’daily health,life ability,improve the quality of life.
