全病程健康管理模式的内涵是将个案管理与全程照护相融合,应用互联网信息技术,对就医流程进行梳理和简化,使传统片段化、分散式的医疗服务形式逐步转变为闭环式、以全人全周期为主的健康管理模式,达到后续可持续优化的效果。包含预约诊疗、线上咨询、双向转诊、个案管理、多元化延伸服务五大服务模块。随着全病程管理模式地提出,其通过不断调整与优化而发展的日趋成熟,大量实践研究可以看出,其在国内的应用取得了一定的效果,但未达到满意的状态。基于此,本文将针对全病程健康管理模式的应用现状、启示进行分析和论述,以期为相关人员提供一定理论指导和参考,促使全病程健康管理模式达到理想的应用成效。

The connotation of whole-course health management model is to integrate case management and whole-course care,apply Internet information technology,sort out and simplify the medical treatment process,and gradually transform the traditional fragmented and decentralized form of medical services into a closed-loop,whole-person,whole-cycle-based health management model,so as to achieve the effect of subsequent sustainable optimization.It contains five service modules:appointment consultation,online consultation,two-way referral,case management,and diversified extended services.As the whole-course management model is proposed,it has been adjusted and optimized continuously and is becoming more and more mature,and a large number of practical studies can show that the domestic application has achieved certain effects,but has not reached a satisfactory state.Based on this,this paper analyzes and discusses the application status and revelation of the whole-course health management model,in order to provide some theoretical guidance and reference for the relevant personnel,and promote the ideal application of the whole-course health management model.
