分级管理预警机制在医院医疗纠纷事件处置中的应用效果

The Effect of the Application of Early Warning Mechanism of Graded Management in the Disposal of Medical Dispute Events in Hospitals

目的:探讨分级管理预警机制在医院医疗纠纷事件处置中的应用效果。方法:选取2019年1月-2022年1月南通市通州区人民医院开设病房的8个手术科室为研究对象,按照随机数字表法分观察组和对照组,各4个科室。对照组采用医疗缺陷管理与医疗纠纷处理相结合管理策略,观察组在对照组基础上采用分级管理预警机制策略。比较两组医疗纠纷发生率、医疗不安全事件发生率、医疗纠纷赔款、医疗安全系数(MSC)。结果:观察组患者医疗纠纷发生率、医疗不安全事件发生率低于对照组,差异有统计学意义(P<0.05);观察组患者每人医疗纠纷赔款低于对照组,差异有统计学意义(P<0.05);2019-2021年,观察组科室MSC值均高于对照组,3年平均MSC值高于对照组,差异有统计学意义(P<0.05)。结论:采取分级管理预警机制模式可显著降低医疗纠纷和医疗不安全事件发生率,提高医院MSC,可成为医院医疗安全管理的一个重要部分。

Objective:To explore the effect of the application of early warning mechanism of graded management in the disposal of medical dispute events in hospitals.Methods:Eight surgical departments of the Tongzhou District People's Hospital,Nantong City,which opened wards from January 2019 to January 2022 were selected for the study,and four departments each were divided into observation and control groups according to the random number table method.The control group adopted a management strategy combining medical defect management and medical dispute handling,and the observation group adopted a graded management early warning mechanism strategy on the basis of the control group.The incidence of medical disputes,the incidence of medical unsafe events,medical dispute payouts,and medical safety coefficient(MSC)were compared between the two groups.Results:The incidence rate of medical disputes and medical insecurity events of patients in the observation group was lower than that of the control group,and the difference was statistically significant(P<0.05).The medical dispute payout per patient in the observation group was lower than that in the control group,and the difference was statistically significant(P<0.05).From 2019 to 2021,the MSC values of the observation group's departments were higher than those of the control group,and the 3-year average MSC values were higher than those of the control group,and the difference was statistically significant(P<0.05).Conclusion:Adopting the model of graded management early warning mechanism can significantly reduce the incidence of medical disputes and medical unsafe events and improve hospital MSC,which can be an important part of hospital medical safety management.
