低剂量扫描结合个体化对比剂用量在CT肺动脉成像中的价值研究 被引量:1

摘要 目的 研究低剂量扫描和个体化对比剂用量相结合在CT肺动脉成像中的应用价值。方法 280例进行CT肺动脉成像检查的患者,根据检查方法不同分为对照组(134例,管电压120 kVp,对比剂为50 ml的370 mg I/ml碘帕醇+30 ml的0.9%氯化钠溶液)及观察组(146例,管电压80 kVp,个体化对比剂用量)。比较两组患者的图像质量和主观评分(肺动脉平均CT值、胸大肌平均CT值、背景噪声、信噪比、对比噪声比、图像质量评分),辐射剂量[容积CT剂量指数(CTDIvol)、剂量长度乘积(DLP)、有效剂量(ED)]。结果 两组患者的肺动脉平均CT值、胸大肌平均CT值、背景噪声、信噪比、对比噪声比、图像质量评分比较,差异无统计学意义(P>0.05)。观察组的CTDIvol(6.21±0.89)mGy、DLP(239.24±1.76)mGy·cm、ED(3.48±0.86)mSv均低于对照组的(12.08±1.35)mGy、(452.46±2.94)mGy·cm、(6.32±1.05)mSv,差异具有统计学意义(P<0.05)。结论 对进行CT肺动脉成像检查的患者利用低剂量扫描结合个体化对比剂用量可以在保证图像检测质量主观评分的基础上减少辐射剂量,有助于提升检查的安全性,值得推荐。 Objective To study the application value of low-dose scanning combined with individualized amount of contrast medium in CT pulmonary artery imaging. Methods A total of 280 patients who underwent CT pulmonary artery imaging were divided into control group(134 patients, tube voltage 120 kVp, contrast medium 50 ml 370 mg I/ml iodopanol+30 ml 0.9% sodium chloride solution) and observation group(146 patients, tube voltage 80 kVp, individualized amount of contrast medium) according to different examination methods. Both groups were compared in terms of image quality and subjective scores(average CT value of pulmonary artery, average CT value of pectoralis major, background noise, signal to noise ratio, contrast to noise ratio, image quality score),radiation dosage [volume CT dose index(CTDIvol), dose length product(DLP), effective dose(ED)]. Results There was no statistically significant difference between the two groups in terms of average CT value of pulmonary artery, average CT value of pectoralis major, background noise, signal to noise ratio, contrast to noise ratio, image quality score(P>0.05). The observation group had CTDIvol of(6.21±0.89) mGy, DLP of(239.24±1.76) mGy·cm and ED of(3.48±0.86) mSv, which were lower than(12.08±1.35) mGy,(452.46±2.94) mGy·cm and(6.32±1.05) m Sv of the control group, and the differences were statistically significant(P<0.05). Conclusion For patients undergoing CT pulmonary artery imaging, low-dose scanning combined with individualized amount of contrast medium can reduce the radiation dose while ensuring the subjective score of image detection quality,which is helpful to improve the safety of the examination and is worthy of recommendation.
