多巴胺与去甲肾上腺素联合参附注射液治疗感染性休克的效果

Effect of Dopamine and Norepinephrine Combined with Shenfu Injection in the Treatment of Septic Shock

目的:探讨多巴胺与去甲肾上腺素联合参附注射液治疗感染性休克的效果。方法:选取2020年1月-2022年1月咸宁市中医医院接受治疗的80例感染性休克患者。根据电脑数字表法分为对照组和观察组,各40例。两组均给予基础治疗,在此基础上,对照组给予血管活性药物治疗,观察组在对照组基础上给予参附注射液。比较两组治疗前后炎症因子与血乳酸水平、血流动力学指标、中医症状评分及相关评分。结果:治疗后,观察组肿瘤坏死因子-α(TNF-α)、白细胞介素-6(IL-6)、血乳酸水平均低于对照组(P<0.05)。治疗后,观察组外周血管阻力指数(SVRI)、心脏指数(CI)均高于对照组,血管外肺水指数(EVLWI)低于对照组(P<0.05)。治疗后,观察组眩晕昏扑、息微唇绪、面色苍白、四肢厥冷评分均低于对照组(P<0.05)。治疗后,观察组序贯器官衰竭评分(SOFA)、急性生理功能和慢性健康状况评分系统Ⅱ(APACHEⅡ)评分均低于对照组(P<0.05)。结论:多巴胺与去甲肾上腺素联合参附注射液治疗感染性休克可有效改善患者炎症因子、血流动力学指标,并缓解相关症状,进一步改善患者预后。

Objective:To investigate the effect of Dopamine and Norepinephrine combined with Shenfu Injection in the treatment of septic shock.Method:A total of 80 patients with septic shock who were treated in Xianning Traditional Chinese Medicine Hospital from January 2020 to January 2022 were selected.They were randomly divided into control group and observation group by computer numerical table method,40 cases in each group.Both groups were given basic treatment.On this basis,the control group was given vasoactive drug treatment,and the observation group was given Shenfu Injection on the basis of the control group.The levels of inflammatory factors and blood lactic acid,hemodynamic indexes,TCM symptom scores and related scores were compared between the two groups before and after treatment.Result:After treatment,the levels of tumor necrosis factor-α(TNF-α),interleukin-6(IL-6)and blood lactic acid in the observation group were lower than those in the control group(P<0.05).After treatment,the systemic vascular resistance index(SVRI)and cardiac index(CI)in the observation group were higher than those in the control group,while the extravascular lung water index(EVLWI)was lower than that in the control group(P<0.05).After treatment,the scores of vertigo,breathing micro lip thread,pale of face,limb jue cold in the observation group were lower than those in the control group(P<0.05).After treatment,the scores of sequential organ failure score(SOFA),acute physiological function and chronic health score systemⅡ(APACHEⅡ)in the observation group were lower than those in the control group(P<0.05).Conclusion:Dopamine and Norepinephrine combined with Shenfu Injection in the treatment of septic shock can effectively improve inflammatory factors of patients,hemodynamic indicators,relieve related symptoms,and further improve the prognosis of patients.
