针刺联合常规训练方案对脑卒中后构音障碍患者言语功能、声学水平及疗效的影响

Effect of Acupuncture Combined with Routine Training on Speech Function,Acoustic Level and Therapeutic Effect in Patients with Dysarthria after Stroke

目的:探讨脑卒中后构音障碍患者采用针刺联合常规训练方案的效果,着重分析其对言语功能、声学水平发挥的改善作用。方法:选择2020年1月-2021年12月就诊于当阳市中医医院的84例脑卒中后构音障碍患者,按照随机号码表法将其分为常规组和针刺组,各42例。常规组给予常规训练方案(语言康复训练),针刺组在常规组基础上另外进行针刺治疗,对比两组的言语功能、声学水平及疗效。结果:治疗后,针刺组汉语失语症检查量表(ABC)各项评分明显高于常规组(P<0.05)。治疗后,针刺组振幅微扰(Shimmer)、声门接触率(CQ)及基频微扰(Jitter)参数明显低于常规组(P<0.05)。治疗后,针刺组最长发声时间(MPT)、篇章正答率及语音清晰度指标明显优于常规组(P<0.05)。针刺组的总有效率为90.48%,明显高于常规组的73.81%(P<0.05)。结论:针刺联合常规训练方案治疗脑卒中后构音障碍,有助于改善患者言语功能及声学水平,并使其整体疗效得以提升。

Objective:To explore the effect of acupuncture combined with routine training on speech function and acoustic level in patients with dysarthria after stroke.Method:A total of 84 patients with dysarthria after stroke treated in Dangyang City Hospital of Traditional Chinese Medicine from January 2020 to December 2021 were selected and divided into routine group and acupuncture group according to the random number table method,with 42 cases in each group.The routine group received routine training(language rehabilitation training),while the acupuncture group received acupuncture treatment at the same time on the basis of the routine group.The speech function,acoustic level and efficacy of the two groups were compared.Result:After treatment,the scores of aphasia battery of Chinese(ABC)in the acupuncture group were significantly higher than those in the routine group(P<0.05).After treatment,Shimmer,contact quotient(CQ)and Jitter parameters in the acupuncture group were significantly lower than those in the routine group(P<0.05).After treatment,the maximum phonation time(MPT),text correct answer rate and speech intelligibility in the acupuncture group were significantly better than those in the routine group(P<0.05).The total effective rate in the acupuncture group was 90.48%,which was significantly higher than 73.81%in the routine group(P<0.05).Conclusion:The combination of acupuncture and routine training in the treatment of dysarthria after stroke is helpful to improve the language function and acoustic level of patients,and improve the overall curative effect.
