壮骨益髓汤对胸腰椎压缩性骨折术后功能恢复、骨代谢指标的作用

Effects of Zhuanggu Yisui Decoction on Functional Recovery and Bone Metabolism Indexes after Thoracolumbar Vertebral Compression Fracture Surgery

目的:探讨壮骨益髓汤在胸腰椎压缩性骨折(thoracolumbar vertebral compression fracture,TLVCF)术后患者中的应用价值。方法:选择2020年1月-2022年1月漳州市第三医院收治的TLVCF术后患者78例,按随机数字表法分为两组,各39例。对照组采用常规治疗,研究组采用壮骨益髓汤治疗,两组均治疗6个月。对比两组术后功能恢复情况、疼痛程度、骨代谢指标、骨愈合情况、不良反应。结果:治疗6个月后,两组Oswestry功能障碍指数问卷表(ODI)评分、视觉模拟评分法(VAS)评分及骨碱性磷酸酶(B-ALP)、Ⅰ型胶原C端肽(CTX-Ⅰ)水平均低于治疗前,骨保护素(OPG)水平及伤椎前缘、中央、后缘高度均高于治疗前,Cobb角均小于治疗前,且研究组均优于对照组,差异均有统计学意义(P<0.05)。两组不良反应比较,差异无统计学意义(P>0.05)。结论:壮骨益髓汤能够减轻TLVCF术后患者疼痛,改善骨代谢指标,加快骨愈合,促进胸腰椎功能恢复,且安全性高。

Objective:To explore the application value of Zhuanggu Yisui Decoction in patients with postoperative thoracolumbar vertebral compression fracture(TLVCF).Method:A total of 78 patients with TLVCF who admitted to Zhangzhou Third Hospital from January 2020 to January 2022 were selected and divided into two groups according to the random number table method,39 cases in each group.The control group was given routine treatment,and the research group was given Zhuanggu Yisui Decoction,two groups were treated for 6 months.The postoperative functional recovery,pain degree,bone metabolism indexes,bone healing and adverse reactions were compared between two groups.Result:After 6 months of treatment,the scores of Oswestry dysfunction index questionnaire(ODI),visual analog scale(VAS),levels of bone alkaline phosphatase(B-ALP)and C-terminal peptide of typeⅠcollagen(CTX-Ⅰ)in two groups were lower than those before treatment,the levels of osteoprotegerin(OPG)and the heights anterior,central and posterior edges of injured vertebra in two groups were higher than those before treatment,and Cobb angles were smaller than those before treatment,the indexes in the research group were better than those of the control group,the differences were statistically significant(P<0.05).The difference of adverse reactions of two groups was not statistically significant(P>0.05).Conclusion:Zhuanggu Yisui Decoction can relieve pain in patients after TLVCF surgery,improve bone metabolism indexes,accelerate bone healing,and promote the recovery of thoracic and lumbar spine function,with high safety.
