酸枣仁汤加减配合耳针治疗肝郁血虚型男性酒依赖性失眠的临床观察

Clinical Observation on Modified Suanzaoren Decoction Combined with Ear Acupuncture in the Treatment of Alcohol Dependent Insomnia in Men with Liver Depression and Blood Deficiency

目的:观察肝郁血虚型男性酒依赖性失眠患者联合应用酸枣仁汤加减与耳针治疗的效果。方法:选取2021年8月-2022年5月于宁德市康复医院物质依赖科进行治疗的肝郁血虚型酒依赖性失眠患者74例,通过随机数字表法分为对照组与观察组,各37例。对照组接受西药治疗,观察组采用酸枣仁汤加减联合耳针治疗,对比两组临床疗效、负性情绪改善情况、脑源性神经营养因子(BDNF)、胶质细胞源性神经营养因子(GDNF)、5-羟色胺(5-HT)、去甲肾上腺素(NE)水平改善情况及治疗后不良反应发生情况。结果:观察组总有效率高于对照组(P<0.05)。两组治疗前SAS、SDS评分、血浆BDNF、GDNF、5-HT及NE水平、中医症候积分比较,差异均无统计学意义(P>0.05);观察组治疗后SAS、SDS评分及早醒、多梦、入睡困难、情绪抑郁及醒后难以入睡评分均低于对照组,血浆BDNF、GDNF、5-HT及NE水平均高于对照组(P<0.05)。观察组不良反应发生率较对照组低(P<0.05)。结论:肝郁血虚型男性酒依赖性失眠患者在采用西药治疗的基础上采用酸枣仁汤加减治疗联合耳针治疗能够提高临床总有效率,改善其负性情绪,加快临床症状缓解,有助于保护患者神经细胞,安全高效。

Objective:To observe the effect of combination of modified Suanzaoren Decoction and ear acupuncture on alcohol dependent insomnia in men with liver depression and blood deficiency.Method:A total of 74 patients with alcohol dependent insomnia of liver depression and blood deficiency type who selected to be treated in the Material Dependence Department of Ningde Rehabilitation Hospital from August 2021 to May 2022 were randomly divided into the control group and the observation group according to the random number table method,37 cases in each group.The control group was treated with western medicine,the observation group was treated with modified Suanzaoren Decoction combined with ear acupuncture.The clinical efficacy,improvement of negative emotions,improvement of brain derived neurotrophic factor(BDNF),glial cell derived neurotrophic factor(GDNF),5-hydroxytryptamine(5-HT),norepinephrine(NE)levels and adverse reactions after treatment of two groups were compared.Result:The total effective rate of the observation group was higher than that of the control group(P<0.05).There were no significant differences in SAS,SDS scores,plasma BDNF,GDNF,5-HT and NE levels,and traditional Chinese medicine symptom scores between two groups before treatment(P>0.05);the SAS,SDS scores,scores of early awakening,dreaminess,difficulty in falling asleep,emotional depression and difficulty in falling asleep in the observation group after treatment were lower than those in the control group,and the plasma BDNF,GDNF,5-HT and NE levels were higher than those in the control group(P<0.05).The incidence of adverse reactions in the observation group was lower than that in the control group(P<0.05).Conclusion:On the basis of western medicine treatment,male alcohol dependent insomnia patients with liver depression and blood deficiency treated with modified Suanzaoren Decoction combined with ear acupuncture can improve the clinical total effective rate,improve their negative emotions,speed up the relief of clinical symptoms,and help protect patients’nerve cells,safe and efficient.
