超敏C-反应蛋白、纤维蛋白原、免疫细胞和调节性T细胞在良性肛周疾病中的表达情况

Expression of hypersensitive C-reactive protein,fibrinogen,immune cells,and regulatory T cells in benign perianal diseases

目的探讨超敏C-反应蛋白(hs-CRP)、纤维蛋白原(FIB)、免疫细胞和调节性T细胞(Treg)在良性肛周疾病中的表达情况。方法选取2020年10月至2021年12月北京市肛肠医院收治的240例肛周疾病患者作为研究对象,按照肛周疾病的类型分为肛周脓肿组(80例)、肛瘘组(80例)和痔疮组(80例),另选取同期的健康志愿者(80名)作为对照组。收集外周血,检测hs-CRP、FIB和中性粒细胞与淋巴细胞比值(NLR),并采用流式细胞术检测淋巴细胞、单核细胞、CD4^(+)T细胞、CD8^(+)T细胞和Treg细胞的比例。结果肛瘘组hs-CRP、FIB的浓度和NLR低于肛周脓肿组,差异有统计学意义(P<0.05);肛瘘组外周血淋巴细胞、单核细胞和Treg细胞的比例、CD4/CD8比值低于肛周脓肿组,差异有统计学意义(P<0.05)。结论hs-CRP、FIB、NLR、淋巴细胞、单核细胞和CD4/CD8比值、Treg细胞等在不同良性肛周疾病中表达不同,可作为潜在的鉴别诊断指标,值得进一步探讨。

Objective To investigate the expression of hypersensitive C-reactive protein(hs-CRP),fibrinogen(FIB),immune cells,and regulatory T cells in benign perianal diseases.Methods A total of 240 patients with perianal disease admitted to Beijing Anorectal Hospital from October 2020 to December 2021 were selected as the research subjects.According to the type of perianal disease,they were divided into perianal abscess group(80 cases),anal fistula group(80 cases)and hemorrhoid group(80 cases),and healthy volunteers(80 cases)in the same period were selected as the control group.Peripheral blood was collected to detect hs-CRP,FIB and neutrophil to lymphocyte ratio(NLR).Flow cytometry was used to detect the proportion of lymphocytes,monocytes,CD4^(+)T cells,CD8^(+)T cells and Treg cells.Results The concentrations of hs-CRP and FIB and NLR in anal fistula group were lower than those in perianal abscess group,with statistically significant differences(P<0.05).The proportion of peripheral blood lymphocytes,monocytes and Treg cells and the CD4/CD8 ratio in the anal fistula group were lower than those in the perianal abscess group,with statistically significant differences(P<0.05).Conclusion hs-CRP,FIB,NLR,lymphocytes,monocytes,CD4/CD8 ratio and Treg cells are expressed differently in different benign perianal diseases,which can be used as potential differential diagnostic indicators and worth further exploration.
