基于深圳1型糖尿病联盟数据的1型糖尿病医疗现状调查

Current medical situation of type 1 diabetes: based on the data from Shenzhen type 1 diabetes mellitus alliance

目的 探讨深圳T1DM联盟(简称深盟T1DM)区域内T1DM医疗现状。方法 选取2018年12月至2021年7月由深盟T1DM登记入中国T1DM联盟协作研究平台网资料完整的首诊T1DM患者346例,按年龄分为儿童组(n=54)、青少年组(n=49)和成人组(n=243),收集各组一般临床资料、生化指标、胰岛自身抗体检测、自我血糖监测、T1DM确诊类型和胰岛素方案等进行对比研究。结果 IA2A检测率和LADA比例成人组高于儿童、青少年组(P<0.05),使用胰岛素泵和经典T1DM比例低于儿童、青少年组(P<0.05)。成人组未监测血糖比例高于儿童组(P<0.05),血糖监测≥8次/d比例低于儿童组。结论 提高深盟T1DM区域T1DM医疗服务能力,加大T1DM自我管理教育与支持力度、规范T1DM慢病管理,提高胰岛自身抗体联合检测率。

Objective To investigate the current medical situation of type 1 diabetes mellitus(T1DM)in Shenzhen type 1 diabetes mellitus alliance(SZT1DM)area.Methods A total of 346patients with T1DM who registered in China Type 1 Diabetes Alliance Collaborative Research Platform Network by SZT1DM were enrolled in this study from December 2018 to July 2021. All the patients were divided into children group(n=54),adolescent group(n=49)and adult group(n=243). The general clinical data,biochemical indexes,detection of islet autoantibodies,self-blood glucose monitoring,T1DM subtypes and insulin regimens were collected and compared among groups.Results The IA2A detection rate and the proportion of latent autoimmune diabetes in adult(LADA)were significantly higher,while the proportion of insulin pump treatment and classical T1DM were significantly lower in adult group than in children and adolescent group(P<0. 05). The proportion of unmonitored blood glucose was higherin adult group than in children group(P<0. 05),and the proportion of blood glucose monitoring ≥8 times/d was lower in adult group than in children group(P<0. 05).Conclusion The medical service ability of T1DM in SZT1DM area needs to be improved. In the future,it is necessary to strengthen T1DM self-management education and support,standardize chronic disease management of T1DM,and expand the detection rate of islet autoantibodies.
