不同孕期孕妇甲状腺激素水平分析及参考区间的研究

Analysis of thyroid hormone levels and reference intervals in pregnant women during different pregnancy periods

目的探讨不同孕期孕妇甲状腺激素水平及参考区间,以期为临床治疗提供参考。方法选取2020年2月—2022年1月于新密市第一人民医院妇产科就诊的344例妊娠妇女作为本研究的观察组,同时选择同期在本院做健康体检的健康非妊娠女性86例作为对照组。比较不同孕期孕妇甲状腺激素水平;确定不同孕期孕妇甲状腺激素水平的参考区间。结果对照组研究对象TT_(3)、TT_(4)水平低于孕早、中、晚期组,而FT_(3)、FT_(4)及TSH水平高于孕早、中、晚期组,差异有统计学意义(P<0.05)。对照组研究对象及孕早期、孕中期、孕晚期孕妇TT_(3)参考区间分别为1.98(1.28,2.84)nmol/L、2.86(1.68,4.36)nmol/L、2.81(1.76,4.31)nmol/L、2.74(1.68,4.19)nmol/L。结论孕妇TSH、FT_(3)与FT等甲状腺激素相关指标不同于非妊娠女性,且不同孕期孕妇体内相关指标也有一定差异,从而建立本地区不同孕期孕妇甲状腺激素参考区间对于本地母婴的健康有积极意义。

Objective To investigate thyroid hormone levels and reference intervals in pregnant women during different pregnancy periods to provide a reference for clinical treatment.Methods A total of 344 pregnant women who visited the Department of Obstetrics and Gynecology of the First People's Hospital of Xinmi from February 2020 to January 2022 were selected as participants in the observation group in this study.Eighty-six healthy non-pregnant women who underwent health examinations in our hospital during the same period were also selected as participants in the control group.This study aimed to compare thyroid hormone levels and to determine the reference interval of thyroid hormone levels in pregnant women during different pregnancy periods.Results The levels of TT_(3)and TT_(4)in the control group were significantly lower than those in the first,second,and third-trimester groups,while the levels of FT_(3),FT_(4),and TSH in the control group were significantly higher than those in the first,second,and third-trimester groups(P<0.05).The reference intervals of TT_(3)in pregnant women were 1.98(1.28,2.84)nmol/L,2.86(1.68,4.36)nmol/L,2.81(1.76,4.31)nmol/L,and 2.74(1.68,4.19)nmol/L in the control,second,and third-trimester groups,respectively.Conclusion Thyroid hormone-related indexes such as TSH,FT_(3),and FT in pregnant women are different from those in non-pregnant women.There are also some differences in related indexes in pregnant women during different pregnancy periods,hence it is of positive importance to establish thyroid hormone reference intervals in pregnant women during different pregnancy periods in this area for the health of local mothers and infants.
