冠状动脉CTA对中间支与左冠状动脉斑块形成相关性的评价

摘要 目的:通过冠状动脉CT血管造影(CT angiography,CTA)初步探讨左冠状动脉中间支(the intermediate branch,IMB)的存在、管径大小与左冠状动脉各分支斑块形成是否具有相关性。方法:从2019年11月-2021年6月我院行冠状动脉CTA检查的心内科住院患者中分别随机选取各31例,分为实验组(有中间支组)31人和对照组(无中间支组) 31人,实验组进一步分为中间支直径大于等于平均值组、直径小于平均值组。在重建图像上分别测量并记录左主干(left main coronary artery,LM)分叉角(前降支/回旋支)、左冠状动脉各支斑块出现情况、狭窄情况及中间支直径,分析中间支的存在、管径大小与左冠状动脉各分支斑块形成之间是否具有相关性。结果:左冠状动脉各支斑块发生率和狭窄阳性发生率与中间支是否存在无显著相关性(P>0.05)。中间支直径与中间支斑块有显著正相关(P<0.05),但与左冠状动脉其余分支粥样斑块无相关(P>0.05)。结论:左冠状动脉中间支的出现并不会对左冠状动脉各分支粥样斑块的形成及狭窄产生影响,仅对其自身斑块形成有影响。 Objective:To investigate the relationship that the exists of intermediate branch(IMB),diameter of IMB with the distribution of plaques in the left coronary by coronary tomographic angiography(CTA).Methods:31 patients were randomly selected in the department of cardiology who underwent coronary CTA examination in our hospital from November 2019 to June 2021.They were divided into the experimental group(with IMB) that 31 patients and the control group(without IMB) that 31 patients.The experimental group was further divided into two groups that the group IMB diameter greater or equal than the mean diameter and the group less than the mean diameter.On the reconstructed images,we measured the bifurcation angle of left main coronary artery(LM),the distribution of plaques and vascular stenosis in the left coronary,the diameter of IMB.The relationship between the exists of IMB,diameter of IMB and the distribution of plaques in the left coronary were analyzed.Results:There was no significant correlation between the distribution of plaque and stenosis in each branch of left coronary artery with the exists of IMB(P>0.05).There was a significant positive correlation between the diameter of IMB and distribution of plaques in the IMB(P0.05).Conclusion:The exists of IMB has no effect on the formation of plaques and vascular stenosis in the left coronary,but it can effect on the formation of plaques in IMB itself.
