益生菌对降低ICU患者呼吸机相关性肺炎发生率的研究 被引量:1

摘要 目的探讨早期服用益生菌对降低呼吸机相关性肺炎发生率的影响。方法收治ICU机械通气患者200例,分为研究组与对照组,对2组的呼吸机相关性肺炎发生率、住ICU时间、住院时间、机械通气时间、28 d死亡率和研究期间的并发症(腹泻、菌血症等)进行比较分析。结果研究组患者呼吸机相关性肺炎的发生率与对照组相比显著降低(P<0.05);研究组患者出现呼吸机相关性肺炎的时间晚于对照组(P<0.05),研究组平均ICU住院时间低于对照组(P<0.05)。治疗后第5天研究组炎症因子水平低于对照组(P<0.05)。研究组住院时间短于对照组(P<0.05)。结论早期鼻饲益生菌可以降低ICU患者VAP的发生率,缩短ICU病房住院时间及机械通气时间,减少患者住院费用,值得临床上广泛使用。 Objective To investigate the effect of early use of probiotics on reducing the incidence of ventilator-associated pneumonia.Methods 200 patients with mechanical ventilation were enrolled and randomly divided into study group and control group.The incidence of ventilator-associated pneumonia,ICU stay,hospital stay,duration of mechanical ventilation,28-day mortality and complications(diarrhea,bacteremia,etc.)during the study were compared and analyzed between two groups.Results incidence of ventilator-associated pneumonia in the study group was significantly lower than that in the control group(P<0.05);The time of ventilator-associated pneumonia in the study group was later than that in the control group(P<0.05),and the average length of ICU stay in the study group was lower than that in the control group(P<0.05).On the 5th day after treatment,the level of inflammatory factors in the study group was lower than that in the control group(P<0.05).The hospitalization time of the study group was shorter than that of the control group(P<0.05).Conclusion early nasal feeding of probiotics can reduce the incidence of VAP in ICU patients,shorten the length of stay in ICU ward and mechanical ventilation time,and reduce the hospitalization expenses of patients,which is worthy of wide clinical use.
