加速康复外科理念框架下的前移护理在贲门癌患者围术期中的应用

Application of Forward-moving Nursing under the Framework of Enhanced Recovery after Surgery Concept in the Perioperative Period of Patients with Cardiac Cancer

摘要 目的:探讨加速康复外科(ERAS)理念框架下的前移护理在贲门癌患者围术期中的应用效果。方法:选取2018年5月—2020年10月郑州大学第一附属医院收治的83例贲门癌患者作为研究对象,按照入院时间不同分为常规组38例和观察组45例。常规组患者采用常规护理干预,观察组患者采用ERAS理念框架下的前移护理。比较两组患者术后机体康复进程、并发症、舒适度(GCQ评分)、护理满意度(SERVQUAL)。结果:观察组患者下床行走时间、住院时间、进食时间、首次排气时间短于常规组患者,差异有统计学意义(t=14.431、23.799、17.203、19.186,P<0.05)。观察组患者生理、心理、社会文化和环境、精神评分高于常规组患者,差异有统计学意义(t=15.973、18.718、9.088、11.699,P<0.05)。观察组患者并发症发生率为2.22%,低于常规组患者的18.42%,差异有统计学意义(χ^(2)=4.487,P<0.05)。观察组患者反应性、有形性、可靠性、保证性、移情性评分高于常规组患者,差异有统计学意义(t=6.863、7.068、14.385、11.133、9.799,P<0.05)。结论:ERAS理念框架下的前移护理应用于贲门癌患者围术期,能提高舒适度,降低并发症发生率,缩短术后康复时间,且护理满意度高。 Objective:To explore the application effect of advanced nursing under the framework of enhanced recovery after surgery(ERAS)in the perioperative period of patients with cardiac cancer.Methods:A total of 83 patients with cardiac cancer who were admitted to the hospital from May 2018 to October 2020 were selected as the research subjects,and were divided into the routine group of 38 cases and the observation group of 45 cases according to the time of admission.The patients in the routine group were treated with routine nursing intervention,and the patients in the observation group were treated with advance nursing under the framework of ERAS concept.The postoperative recovery process,complications,comfort(GCQ score)and nursing satisfaction(SERVQUAL)were compared between the two groups.Results:The time of getting out of bed,walking time,hospitalization time,eating time,and first exhaust time in the observation group were shorter than those in the routine group,and the differences were statistically significant(t=14.431,23.799,17.203,19.186,P<0.05).The physical,psychological,social culture and environment,and mental scores of the observation group were higher than those of the routine group,and the difference was statistically significant(t=15.973,18.718,9.088,11.699,P<0.05).The incidence of complications in the observation group was 2.22%,which was lower than 18.42%in the routine group,and the difference was statistically significant(χ^(2)=4.487,P<0.05).The scores of reactivity,tangibility,reliability,assurance,and empathy in the observation group were higher than those in the routine group,and the difference was statistically significant(t=6.863,7.068,14.385,11.133,9.799,P<0.05).Conclusion:Advance nursing under the framework of ERAS concept is applied in the perioperative period of patients with cardiac cancer,which can improve comfort,reduce the incidence of complications,shorten postoperative recovery time,and provide high nursing satisfaction.
