玻璃体腔注射雷珠单抗治疗糖尿病视网膜病变伴黄斑水肿的疗效分析

Efficacy analysis of intravitreal injection of ranibizumab in the treatment of diabetes retinopathy with macular edema

目的 观察玻璃体腔注射雷珠单抗治疗糖尿病视网膜病变(DR)伴黄斑水肿的疗效。方法 60例DR伴黄斑水肿患者为研究对象,均接受玻璃体腔注射雷珠单抗治疗。比较治疗前及治疗后1个月的最佳矫正视力、黄斑中心凹视网膜厚度(CMT)、黄斑中心凹下脉络膜厚度,统计术后并发症发生情况。结果 治疗后1个月的最佳矫正视力(0.51±0.09)显著高于治疗前的(0.26±0.06),CMT(331.67±79.35)μm及黄斑中心凹下脉络膜厚度(191.37±25.34)μm均小于治疗前(563.73±86.67)、(213.96±43.77)μm,差异有统计学意义(P<0.05)。60例患者治疗后,2例出现轻微眼压升高,经对症治疗后好转,未见葡萄膜炎、视网膜脱离及视网膜裂孔等严重并发症,并发症发生率为3.33%。结论 对DR伴黄斑水肿患者使用玻璃体腔注射雷珠单抗治疗可显著提升最佳矫正视力,减少CMT及黄斑中心凹下脉络膜厚度,安全性较高。

Objective To observe the efficacy of intravitreal injection of ranibizumab in the treatment of diabetes retinopathy(DR) with macular edema.Methods 60 patients with DR and macular edema were selected for the study,all of whom received intravitreal injection of ranibizumab.The best corrected visual acuity,central macular thickness(CMT) and sub-foveal choroidal thickness before and 1 month after treatment were compared,and the occurrence of postoperative complications were counted.Results At 1 month after treatment,the best corrected visual acuity(0.51±0.09) was significantly higher than(0.26±0.06) before treatment;the CMT(331.67±79.35) μm and sub-foveal choroidal thickness(191.37±25.34) μm were smaller than(563.73±86.67)and(213.96±43.77) μm before treatment;the differences were statistically significant(P<0.05).2 of the 60 patients had slight elevated intraocular pressure after treatment,which improved after symptomatic treatment,and no serious complications related to uveitis,retinal detachment and retinal tears were found.The overall incidence of complications was 3.33%.Conclusion For patients with DR and macular edema,intravitreal injection of ranibizumab can significantly improve the best corrected visual acuity,reduce CMT and sub-foveal choroidal thickness,and the treatment is safe.
