阿戈美拉汀联合盐酸舍曲林治疗伴有失眠症状的老年抑郁症患者的疗效观察

Efficacy observation of agomelatine combined with sertraline hydrochloride in the treatment of elderly depressed patients with insomnia symptoms

目的 观察阿戈美拉汀联合盐酸舍曲林治疗伴有失眠症状的老年抑郁症患者的疗效。方法 50例伴有失眠症状的老年抑郁症患者,按照治疗方法不同分为对照组和实验组,每组25例。对照组单独采用盐酸舍曲林治疗,实验组采用阿戈美拉汀联合盐酸舍曲林治疗。比较两组治疗前后抑郁症状和失眠质量评分、血清氧化应激指标和神经递质相关指标及不良反应发生情况。结果 治疗8周后,两组汉密尔顿抑郁量表(HAMD)评分均低于本组治疗前,且实验组HAMD评分(11.32±1.25)分低于对照组的(14.34±1.28)分,差异有统计学意义(P<0.05)。治疗8周后,两组匹兹堡睡眠质量指数(PSQI)评分均低于本组治疗前,且实验组PSQI评分(6.38±1.28)分低于对照组的(11.35±1.24)分,差异有统计学意义(P<0.05)。治疗8周后,两组超氧化物歧化酶(SOD)水平高于本组治疗前,丙二醛(MDA)水平低于本组治疗前,且实验组SOD(95.43±3.25)U/ml高于对照组的(78.21±3.29)U/ml,MDA(8.25±1.35)μmol/L低于对照组的(11.25±1.32)μmol/L,差异有统计学意义(P<0.05)。治疗8周后,两组多巴胺(DA)、5羟色胺(5-HT)、去甲肾上腺素(NE)均高于本组治疗前,且实验组DA(213.53±5.48)μg/L、5-HT(352.61±28.42)μg/L、NE(71.48±2.55)ng/L均高于对照组的(174.32±5.42)μg/L、(304.42±28.48)μg/L、(53.28±2.58)ng/L,差异有统计学意义(P<0.05)。两组不良反应发生率比较,差异无统计学意义(P>0.05)。结论 伴有失眠症状的老年抑郁症患者采用阿戈美拉汀联合盐酸舍曲林治疗效果显著优于单独应用盐酸舍曲林治疗。

Objective To observe the efficacy of agomelatine combined with sertraline hydrochloride in the treatment of elderly depressed patients with insomnia symptoms.Methods A total of 50 elderly depressed patients with insomnia symptoms were divided into a control group and an experimental group according to different treatment methods,with 25 cases in each group.The control group was treated with sertraline hydrochloride alone,and the experimental group was treated with agomelatine combined with sertraline hydrochloride.The depressive symptoms,quality of insomnia score,serum oxidative stress indicators and neurotransmitter related indicators before and after treatment,and the occurrence of adverse reactions were compared between the two groups.Results After 8 weeks of treatment,the Hamilton depression scale(HAMD) scores in both groups were lower than those before treatment in this group,and the HAMD score in the experimental group was(11.32±1.25) points,which was lower than(14.34±1.28) points in the control group,and the differences were statistically significant(P0.05).Conclusion Agomelatine combined with sertraline hydrochloride has better effect than sertraline hydrochloride alone for elderly depressed patients with insomnia symptoms.
