舒洛地特软胶囊对带隧道和涤纶套透析导管通畅性的疗效分析

Effect analysis of sulodexide soft capsules on patency of tunneled-cuffed catheter

目的 探讨舒洛地特软胶囊对带隧道和涤纶套透析导管(TCC)通畅性的影响及安全性。方法 采取随机数字表法对2020年6月~2021年6月在南通大学附属瑞慈医院血液净化中心使用TCC行维持性血液透析治疗的患者40例分为对照组(n=20)和观察组(n=20)。对照组患者常规血液透析,观察组在对照组基础上,非透析日口服舒洛地特软胶囊250LSU,每日2次,两组患者观察3个月,对比干预后两组透析血流量、出血事件、导管感染发生率及血红蛋白(Hb)、血小板(PLT)、出凝血时间(PT、APTT)、C反应蛋白(CRP)水平。结果 实施3个月后,观察组的透析血流量显著高于对照组(P <0.05);观察组的C反应蛋白(CRP)显著低于对照组(P <0.05)。结论 舒洛地特可以显著改善TCC通畅性,有效提高透析血流量,且未增加出血事件及导管感染发生率,促进改善机体微炎症状态。

Objective To investigate the effect and safety of Sulodex soft capsule on patency of dialysis catheter with tunnel and dacron jacket(TCC).Methods 40 patients receiving maintenance hemodialysis using TCC in Blood Purification Center of Ruici Hospital affiliated to Nantong University from June 2020 to June 2021 were divided into the control group(n = 20) and the observation group(n = 20) by random number table method. The control group was routinely hemodialysis, and the observation group was given sulodexide soft capsule 250LSU orally on non-dialysis days on the basis of the control group,1 day 2 times. The two groups were observed for 3 months. Hemodialysis blood flow, bleeding events, incidence of catheter infection, hemoglobin(Hb), platelet(PLT), coagulation time(PT, APTT) and C-reactive protein(CRP) levels were compared between the two groups after intervention. Results 3 months after implementation, the hemodialysis blood flow of the observation group was significantly higher than that of the control group(P < 0.05). The C-reactive protein(CRP) in the observation group was significantly lower than that in the control group(P < 0.05). Conclusion Sulodexide can significantly improve the patency of TCC, effectively increase hemodialysis blood flow, without increasing the incidence of bleeding events and catheter infection, and promote the improvement of microinflammation.
