摘要
目的 探讨醒脑静注射液联合银杏叶提取物治疗脑卒中后认知功能障碍疗效及其对血清BDNF和NGF水平的影响。方法 研究2019年2月~2022年2月治疗的150例脑卒中伴有认知功能障碍患者的病例资料,按随机数字表法将患者分为A组(n=50)、B组(n=48)和C组(n=52),A组给予醒脑静注射液静脉滴注,B组给予银杏叶提取物静脉滴注,C组在A组的基础上联合使用银杏达提取物注射液。治疗2周之后,比较三组患者的治疗效果,NIHSS、ADL评分以及MMSE的改善情况,血清BDNF、NGF水平、脑内神经递质的变化以及药物不良反应的发生情况。结果 C组治疗的总有效率(94.23%)高于A组(80.00%)、B组(77.08%),差异有统计学意义(P <0.05);三组治疗后的NIHSS比治疗前低,ADL、MMSE比治疗前高,且治疗后C组的NIHSS比A、B组低,ADL、MMSE比A、B组高(P <0.05);三组治疗后的NIHSS比治疗前低,ADL、MMSE比治疗前高,且治疗后C组的NIHSS比A、B组低,ADL、MMSE比A、B组高(P <0.05);三组患者治疗后的5-HT、DA、NE水平均比治疗前高,且治疗后C组的5-HT、DA、NE水平比A、B组高(P <0.05)。结论 醒脑静注射液联合银杏叶提取物治疗脑卒中后认知功能障碍疗效明确,可有效地改善患者的脑功能缺损问题,降低患者的血清BDNF和NGF水平,提高患者的生活自理能力。
Objective To investigate the effect of Xingnaojing Injection combined with ginkgo biloba extract on cognitive dysfunction after stroke and its effect on serum BDNF and NGF levels. Methods A prospective study was conducted on the pathological data of 150 stroke patients with cognitive impairment treated in our hospital from February 2019 to February 2022. The patients were divided into group A(n = 50), group B(n = 48) and group C(n = 52) according to random number table. Group A was given Xingnaojing Injection intravenous drip, group B was given ginkgo biloba extract intravenous drip, and group C was combined with ginkgo biloba extract injection on the basis of group A. After two weeks of treatment, the treatment effect, NIHSS, ADL score and MMSE improvement, serum BDNF, NGF level, brain neurotransmitter changes and adverse drug reactions were compared among the three groups.Results The total effective rate of group C( 94.23%) was higher than that of group A( 80.00%) and group B(77.08%), and the difference was statistically significant( P< 0.05). The NIHSS of the three groups after treatment was lower than that before treatment, ADL and MMSE were higher than those before treatment, and the NIHSS of group C after treatment was lower than that of group A and group B, ADL and MMSE were higher than those of group A and group B(P < 0.05).The NIHSS of the three groups after treatment was lower than that before treatment, ADL and MMSE were higher than those before treatment, and the NIHSS of group C after treatment was lower than that of group A and group B, ADL and MMSE were higher than those of group A and group B(P < 0.05). After treatment, the levels of 5-HT, DA and NE in the three groups were higher than those before treatment, and the levels of 5-HT, DA and NE in group C were higher than those in group A and B(P < 0.05).Conclusion Xingnaojing Injection combined with ginkgo biloba leaf extract has clear efficacy in the treatment of cognitive dysfunction after stroke, which can effectively improve the brain function defect of patients, reduce the levels of serum BDNF and NGF, and improve the self-care ability of patients.
