胜湿止痒方治疗急性肛门湿疹的疗效及对中医症状积分、肛门功能、血清炎性因子水平的影响

Curative effect of Shengshi Zhiyang Recipe on acute anal eczema and its influence on TCM symptom score, anal function and serum inflammatory factor level

目的 观察胜湿止痒方应用于急性肛门湿疹(Eczema of anus,EA)的效果。方法 选取2021年1月~2022年2月肛肠科收治的120例EA患者,按随机数字表法进行分组,对照组60例,予以糠酸莫米松乳膏治疗,治疗组60例,在对照组基础上予以胜湿止痒方,7 d为1个疗程,共治疗2个疗程。比较两组治疗前、治疗2个疗程后中医症状积分(肛周湿疹面积、肛周渗液、瘙痒程度、湿疹形态)、肛门功能(直肠静息压、肛管静息压、肛管最大收缩压)、血清炎性因子水平[白细胞介素-4(IL-4)、白细胞介素-5(IL-5)、肿瘤坏死因子-α(TNF-α)]水平,并统计两组疗效、治疗后3个月复发率。结果 治疗组治疗总有效率较对照组高,复发率较对照组低(P <0.05);治疗后,两组肛周湿疹面积、肛周渗液、瘙痒程度、湿疹形态积分均降低,治疗组较对照组低(P <0.05);两组治疗2个疗程后直肠静息压、肛管静息压、肛管最大收缩压均升高,治疗组高于对照组(P<0.05);治疗2个疗程后,治疗组与对照组血清炎性因子水平均改善,且治疗组改善程度高于对照组(P <0.05)。结论 胜湿止痒方治疗EA疗效显著,能提升肛门功能,下调血清炎性因子水平,改善临床症状,降低复发风险。

Objective To observe the effect of Shengshi Zhiyang Recipe on acute anal eczema of anus(EA). Methods 120 patients with EA treated in Jimo District Hospital of traditional Chinese medicine in Qingdao, Shandong Province from January 2021 to Februray 2022 were randomly divided into two groups. 60 patients in the control group were treated with mometasone furoate cream, and 60 patients in the treatment group were treated with Shengshi Zhiyang Recipe on the basis of the control group. Seven days was a course of treatment, a total of two courses of treatment. The TCM symptom scores(perianal eczema area, perianal exudation, pruritus degree, eczema morphology), anal function(rectal resting pressure, anal resting pressure, anal maximum systolic pressure), serum inflammatory factor levels [interleukin-4(IL-4), interleukin-5(IL-5), tumor necrosis factor] were compared between the two groups before treatment and after two courses of treatment-α(TNF-α).The curative effect of the two groups and the recurrence rate of 3 months after treatment were counted. Results The total effective rate of the treatment group was higher than that of the control group, and the recurrence rate was lower than that of the control group(P < 0.05);After treatment, the area of perianal eczema, perianal exudation, pruritus and eczema morphological score in the two groups were decreased, and the treatment group was lower than the control group(P < 0.05);After two courses of treatment, the rectal resting pressure, anal resting pressure and anal maximum systolic pressure in the treatment group were higher than those in the control group(P < 0.05);After two courses of treatment, the levels of serum inflammatory factors in the treatment group and the control group were improved, and the improvement degree in the treatment group was higher than that in the control group(P < 0.05). Conclusion Shengshi Zhiyang Recipe has a significant effect on EA, which can improve anal function, reduce the level of serum inflammatory factors, improve clinical symptoms and reduce the risk of recurrence.
