摘要
目的探讨亚低温治疗对重症颅脑损伤患儿早期大脑氧代谢的影响。方法回顾性分析2020年1月至2021年12月湖南省儿童医院收治的84例重症颅脑损伤患儿的临床资料,根据是否采用亚低温治疗分为对照组(24例)和观察组(60例),对照组行常规治疗,观察组予以常规治疗联合亚低温治疗,分析两组患儿早期大脑氧代谢的改变。结果与治疗前比较,两组患儿治疗后24、48 h颅内压水平更低,且观察组低于对照组,差异有统计学意义(P<0.05);两组患儿治疗后1周的血氧含量、颈静脉球部血氧饱和度、脑氧摄取率及动脉血氧含量水平均高于治疗前,且观察组高于对照组,差异有统计学意义(P<0.05);与治疗前比较,两组患儿治疗后1周的格拉斯哥昏迷量表(GCS)评分均提高,且观察组分值高于对照组,差异有统计学意义(P<0.05);两组患儿的并发症发生率比较差异无统计学意义(P>0.05)。结论亚低温治疗可促进重症颅脑损伤患儿早期颅内压的下降,并有助于改善其脑氧代谢,缓解患儿的意识障碍,且不会增加并发症。
Objective To explore the effect of mild hypothermia therapy on early cerebral oxygen metabolism in children with severe craniocerebral injury.Methods The clinical data of 84 children with severe craniocerebral injury admitted to Hunan Provincial Children's Hospital from January 2020 to December 2021 were retrospectively analyzed.They were divided into control group(n=24)and observation group(n=60)according to whether mild hypothermia therapy was given.Control group received conventional treatment,and the observation group was given conventional treatment plus mild hypothermia treatment.The changes of early cerebral oxygen metabolism of two groups were analyzed.Results When compared to before treatment,the intracranial pressures in two groups at 24h and 48h after treatment were decreased,and their levels in observation group were lower than those in control group(P<0.05).The levels of blood oxygen,jugular bulb oxygen saturation,cerebral oxygen uptake rate and arterial oxygen content in two groups at 1 week after treatment were higher than those before treatment,and they were higher in observation group than those in control group(P<0.05).When compared to before treatment,Glasgow Coma Scale(GCS)in two groups were increased at 1 week after treatment,and GCS in observation group was higher than that in control group(P<0.05).There was no significant difference in the incidence of complications between two groups(P>0.05).Conclusion Mild hypothermia therapy may decrease the early intracranial pressure in children with severe craniocerebral injury,improve their cerebral oxygen metabolism,alleviate their consciousness disorders and do not increase complications.
