定量与定性筛查在早期胃癌筛查中的效果分析

Application Effect of Quantitative and Qualitative Screening in Early Gastric Cancer Screening

目的探讨定量与定性筛查模式在早期胃癌筛查中的应用价值。方法回顾性选取早期胃癌患者62例作为早癌组,良性胃病患者138例作为良性组,体检健康人员50例作为健康组,检测胃蛋白酶原1(PG1)、PG2、胃泌素17(G17)、幽门螺旋杆菌抗体(Hp-IgG)。结果PG1水平中,早癌组低于良性组,良性组低于健康组,Hp-IgG抗体、PG2、G17水平中,早癌组高于良性组,良性组高于健康组(F=18.642、23.476、26.752、42.762,均p<0.001)。PG1、PG2、G17异常率及Hp-IgG抗体阳性率中,早癌组高于良性组,良性组高于健康组(χ~2=92.659、80.956、73.744、49.323,均p<0.001)。PG1、PG2、G17联合Hp-IgG抗体筛查早期胃癌的敏感度、特异度、准确度及Kappa值分别为96.77%、89.36%、91.20%、0.905。结论定量与定性筛查模式在早期胃癌筛查中具有良好的应用价值。

Objective To discuss the application value of quantitative and qualitative screening in early gastric cancer screening.Methods 62 patients with early gastric cancer were retrospectively selected as early cancer group,138 patients with benign gastric disease were selected as benign group,and 50 healthy persons were selected as healthy group.Pepsinogen 1(PG1),PG2,gastrin 17(G17),and helicobacter pylori antibody (Hp-IgG) were detected.Results The PG1 level in the early cancer group was lower than that in the benign group,and the benign group was lower than that in the healthy group.The levels of Hp-IgG,PG2 and G17 in the early cancer group were higher than those in the benign group,and those in the benign group were higher than those in the healthy group (F=18.642,23.476,26.752,42.762,all p<0.001).The abnormal rates of PG1,PG2 and G17 and the positive rates of Hp-IgG in the early cancer group were higher than those in the benign group,and those in the benign group were higher than those in the healthy group (χ~2=92.659,80.956,73.744,49.323,all p<0.001).The sensitivity,specificity,accuracy and Kappa values of PG1,PG2 and G17 combined with Hp-IgG for screening early gastric cancer were 96.77%,89.36%,91.20%and 0.905,respectively.Conclusion Quantitative and qualitative screening models have good application value in early gastric cancer screening.
