综合老年学评估在透析患者中的应用

Application of Comprehensive Gerontics Assessment in Dialysis Patients

目的:研究综合老年学评估运用于透析患者中的预测价值。方法:选择九江市第一人民医院肾内科2017年8月-2021年9月收治的透析患者70例,按随机数字表法分成两组,各35例。研究组接受综合老年学评估,对照组接受常规评估。干预前后开展简易精神状态量表(MMSE)、Morse跌倒评估量表(MFS)、微型营养评定法(MNA-SF)、老年抑郁量表(GDS-15)、健康调查简表(SF-36)调查,干预后测评两组满意度,并统计并发症(出血、透析失衡综合征、皮肤感染、低血压)。结果:干预前两组MMSE、MFS、MNA-SF、GDS-15评分比较,差异均无统计学意义(P>0.05),干预后研究组MMSE、MNA-SF评分均高于对照组,MFS、GDS-15评分均低于对照组(P<0.05)。研究组满意度为94.29%,高于对照组的71.43%(P<0.05)。研究组并发症发生率为8.57%,低于对照组的34.29%(P<0.05)。干预前两组SF-36各项评分比较,差异均无统计学意义(P>0.05),干预后研究组活力、生理功能、情感职能、心理健康评分均高于对照组(P<0.05)。结论:综合老年学评估有助于消除透析患者负性情绪,增强认知,提高营养水平,患者满意度高,同时可减少并发症发生,安全性高。

Objective:To study the predictive value of comprehensive gerontics assessment in dialysis patients.Method:A total of 70 dialysis patients admitted to the Department of Nephrology,Jiujiang NO.1 People’s Hospital from August 2017 to September 2021 were selected and divided into two groups according to the random number table method,35 cases in each group.The research group received comprehensive gerontics assessment,and the control group received routine assessment.Before and after intervention,mini-mental state examination(MMSE),Morse fall scale(MFS),mini-nutritional assessment(MNA-SF),geriatric depression scale-15(GDS-15)and the MOS item short from health survey(SF-36) were performed.After intervention,the satisfaction of the two groups were evaluated,and the complications(bleeding,dialysis imbalance syndrome,skin infection,hypotension)were counted.Result:Before intervention,the differences in scores of MMSE,MFS,MNA-SF and GDS-15 between the two groups were not statistically significant(P>0.05);after intervention,the scores of MMSE and MNA-SF in the research group were higher than those in the control group,and the scores of MFS and GDS-15 were lower than those in the control group(P<0.05).The satisfaction of the research group was 94.29%,which was higher than 71.43% of the control group(P<0.05).The complication rate of the research group was 8.57%,which was lower than 34.29%of the control group(P<0.05).Before intervention,there were no significant differences in various items of SF-36 scores between the two groups(P>0.05).After intervention,the vitality,physiological function,emotional function and mental health scores in the research group were higher than those in the control group(P<0.05).Conclusion:Comprehensive gerontics assessment can help to eliminate negative emotions of dialysis patients,enhance cognition,and improve nutrition level,resulting in high patient satisfaction and reduction of complications with high safety.
