彩色多普勒超声发现螺旋层流在颈动脉狭窄血液动力学中价值 被引量:1

摘要 目的应用彩色多普勒超声(CDFI)观察颈内动脉斑块周边狭窄后的螺旋层流(HBF),分析其与血液动力学的相关性,探讨HBF作为颈动脉斑块形成预测及狭窄程度评估的可行性。方法对41例颈内动脉狭窄患者和49例健康志愿者进行颈内动脉螺旋层流检查。通过颈内动脉横断面彩色多普勒超声检查,观察是否存在螺旋层流。在颈内动脉纵、横切面运用脉冲多普勒成像,对螺旋层流进行定量评价。结果研究组与对照组中颈内动脉均发现螺旋层流。研究组螺旋层流占比82.9%,对照组螺旋层流占比81.60%。研究组颈内动脉狭窄后螺旋层流以单螺旋最多见(56.1%),双螺旋次之(26.8%);对照组颈内动脉螺旋层流以单螺旋最多见(48.9%),双螺旋次之(32.7%)。研究组颈内动脉流速与螺旋层流旋转流速均高于对照组。研究组与对照组颈内动脉流速与螺旋层流旋转流速呈正相关(P<0.005)。结论颈内动脉内螺旋层流运动模式是人体内存在的正常生理血液流动模式,随着血管内轴向流速增加,旋转流速也增加。螺旋层流在颈内动脉狭窄中起保护作用,可作为颈动脉斑块形成的风险预估因素。
