醒脑开窍针法对改善脑垂体瘤术后持续性头痛的效果及机制研究

Study on the effect and mechanism of activating brain and opening orifices acupuncture in improving persistent headache after pituitary tumor surgery

目的探讨醒脑开窍针法治疗脑垂体瘤术后持续性头痛的临床价值。方法选取2021年3月至2022年3月南阳市中心医院86例脑垂体瘤术后持续性头痛患者,随机数字表分为对照组与研究组,各43例。对照组患者接受常规治疗,根据患者病情实际情况给予对乙酰氨基酚片、甘露醇注射液或丙硫氧嘧啶片治疗,研究组联合醒脑开窍针法治疗。比较两组的中医证候积分、头痛影响测试量表-6(HIT-6)评分、临床疗效、头痛视觉模拟评分(VAS)、生活质量量表(SF-36)评分、头痛发作频率与持续时间。结果治疗前,两组的中医证候积分和HIT-6、SF-36、VAS评分差异无统计学意义(P>0.05);治疗后,研究组的中医证候积分和HIT-6、VAS评分低于对照组(P<0.05),SF-36评分高于对照组(P<0.05)。研究组的头痛发作频率小于对照组(P<0.05),疗效总有效率(97.67%VS 81.40%)高于对照组(P<0.05),头痛持续时间短于对照组(P<0.05)。结论醒脑开窍针法能够有效减轻脑垂体瘤切除术后持续性头痛患者头痛感和负面影响,降低头痛频率,改善生活质量。

Objective To explore the clinical value of Xingnao Kaiqiao acupuncture in treating persistent headache after pituitary tumor surgery.Methods A total of 86 patients with persistent headache after pituitary tumor surgery in Nanyang Central Hospital from March 2021 to March 2022 were selected and randomly divided into control group and study group,with 43 cases in each group.The control group received conventional treatment,and they were given acetaminophen tablets,mannitol injection or propyl thiouracil tablets treatment according to the actual conditions of patients.The study group combined with additional Xingnao Kaiqiao acupuncture treatment.TCM syndrome score,headache impact test scale-6(HIT-6)score,clinical efficacy,visual analogue scale for headache(VAS),quality of life scale(SF-36)score,frequency and duration of headache were compared between the two groups.Results Before treatment,TCM syndrome scores and HIT-6,SF-36 and VAS scores were not significantly different between the two groups(P>0.05).After treatment,TCM syndrome scores,HIT-6 and VAS scores of the study group were lower than those of the control group(P<0.05),and SF-36 scores were higher than those of the control group(P<0.05).The incidence of headache in the study group was lower than that in the control group(P<0.05),the total effective rate(97.67%VS 81.40%)was higher than that in the control group(P<0.05),and the duration of headache was shorter than that in the control grou(P<0.05).Conclusion Xingnao Kaiqiao acupuncture can effectively alleviate the headache and negative effects of patients with persistent headache after pituitary tumor resection,reduce the frequency of headache and improve the quality of life.
