正五聚体蛋白3和C反应蛋白与EB病毒合并肺炎支原体感染所致社区获得性肺炎患儿预后的相关性

Correlation of PTX3 and CRP with prognosis of children with community-acquired pneumonia caused by EB virus and Mycoplasma pneumoniae infection

目的探讨正五聚体蛋白3(PTX3)、C反应蛋白(CRP)与EB病毒合并肺炎支原体感染所致社区获得性肺炎患儿预后的相关性。方法选取2020年5月至2021年5月在我院住院的社区获得性肺炎(CAP)患儿102例为研究对象,同时选取我院儿保科门诊年龄、性别与CAP患儿相仿的健康体检儿童50例作为对照组。根据CAP患儿病情严重度分为轻度组、重度组,其中轻度组48例,重度组54例。对患儿进行常规治疗,根据患儿预后情况分为预后良好组和预后不良组,对各组研究对象血清PTX3、CRP水平进行检测,采用多元线性回归进行相关性分析,采用ROC曲线评价诊断效果。结果3组对象PTX3、CRP水平比较差异均有统计学意义(均P<0.05),其中PTX3、CRP水平均对照组<轻度组<重度组;预后良好组患儿PTX3、CRP水平均低于预后不良组(均P<0.05)。PTX3、CRP与EB病毒合并肺炎支原体感染所致CAP患儿的预后均存在相关性(均P<0.05)。PTX3、CRP诊断EB病毒合并肺炎支原体感染所致CAP患儿预后的敏感度分别为96.70%、96.70%,特异度分别为97.20%、95.80%。ROC曲线分析显示,PTX3、CRP诊断EB病毒合并肺炎支原体感染所致CAP患儿预后的AUC分别为0.990(95%CI:0.973~1.000)、0.940(95%CI:0.860~1.000)。结论EB病毒合并肺炎支原体感染所致CAP患儿的血清PTX3、CRP水平与其病情存在紧密关系,PTX3、CRP对患儿的预后也会产生不同程度的影响。PTX3、CRP在评估EB病毒合并肺炎支原体感染所致CAP预后中的价值较高。

Objective To explore the correlation of positive pentamer protein 3(PTX3)and C-reactive protein(CRP)with the prognosis of children with community-acquired pneumonia(CAP)caused by Epstein Barr(EB)virus and Mycoplasma pneumoniae(MP)infection.Methods A total of 102 children with CAP hospitalized in our hospital from May 2020to may 2021 were enrolled as the subjects;50 healthy children of the similar age and gender receiving physical examination in our hospital were enrolled as the controls.According to the severity of the disease,the children with CAP were divided into mild group(n=48)and severe group(n=54),and all the children received the conventional treatment.According to the prognosis,the children were divided into good prognosis group and poor prognosis group,and their serum PTX3and CRP levels were detected.The correlation analysis was performed using multiple linear regression analysis,and the diagnostic effect was evaluated by ROC curve.Results There were significant differences in the levels of PTX3 and CRP among the three groups(P<0.05),and these levels from low to high was the control group,mild group and severe group.The levels of PTX3 and CRP in the good prognosis group were lower than those in the poor prognosis group(all P<0.05).There was a correlation of PTX3 and CRP with the prognosis of children with CAP caused by EB virus and MP infection(all P<0.05).The sensitivities of PTX3 and CRP in the diagnosis of CAP caused by EB virus and MP infection were96.70%and 96.70%respectively,and the specificities were 97.20%and 95.80%respectively.ROC curve analysis showed that the AUCs of PTX3 and CRP in the diagnosis of CAP caused by EB virus and MP infection were 0.990(95%CI:0.973-1.000)and 0.940(95%CI:0.860-1.000)respectively.Conclusion There is a close relationship of serum PTX3 and CRP levels with condition of children with CAP caused by EB virus and MP infection.PTX3 and CRP also have different degrees of effect on the prognosis of the children,which are of high value to evaluate the prognosis of CAP caused by EB virus and MP infection.
