扩创植皮术联合VSD技术治疗四肢深度烧伤 被引量:4

摘要 目的探讨扩创植皮术联合封闭负压引流(Vacuum sealing drainage,VSD)技术结合纳米银敷料与生长因子对四肢深度烧伤创面愈合的影响。方法选取2017年1月-2020年12月收治的80例四肢深度烧伤患者,采用随机数字表法分为对照组与观察组,每组各40例。对照组行扩创植皮术后,予重组牛碱性成纤维细胞成长因子及纳米银敷料治疗;观察组在对照组基础上联合VSD技术,并比较两组治疗效果。结果观察组总有效率为97.50%,明显高于对照组的80.00%(P<0.05);观察组第一次换药、第二次换药、第三次换药数字疼痛量表(NRS)评分均明显低于对照组(P<0.05);观察组创面愈合时间、住院天数明显短于对照组(P<0.05);观察组并发症发生率为5.00%,明显低于对照组的20.00%(P<0.05)。结论扩创植皮术联合VSD技术,并结合纳米银敷料与生长因子治疗四肢深度烧伤创面效果良好,能够有效促进创面愈合。 Objective To explore the effects of vacuum sealing drainage(VSD)combined with nanometer silver dressing and growth factor on wound healing in patients with deep limbs burns during debridement and skin graft.Methods Eighty patients with deep limbs burns were enrolled between January 2017 and December 2020.They were divided into control group and observation group by random number table method,40 cases in each group.The control group underwent debridement and skin graft,while observation group was additionally treated with VSD.The surgical curative effect was compared between two groups.Results The total response rate was significantly higher in observation group than control group(97.50%vs 80.00%)(P<0.05).The scores of numerical rating scale(NRS)at the first,second and third dressing changes were significantly lower in observation group than control group(P<0.05).The wound healing time and hospitalization time in observation group were shorter than those in control group(P<0.05).The incidence of complications was significantly lower in observation group than control group(5.00%vs 20.00%)(P<0.05).Conclusion The effect of debridement and skin graft combined with VSD is good on deep limbs burns,which can effectively promote wound healing.
