下睑眶隔脂肪移植修复术治疗重睑成形术后不良形态效果分析

Analysis of the Effect of Fat Grafting on Orbital Septum of Lower Eyelid in the Treatment of Poor Morphology after Double Eyelid Blepharoplasty

目的探讨下睑眶隔脂肪移植修复术在重睑成形术后不良形态中的应用效果。方法给予2019年10月至2021年10月潍坊医学院整形外科医院收治的60例重睑成形术后不良形态患者行下睑眶隔脂肪移植修复术治疗,观察眼睑形态、重睑线凹陷过深分级、患者满意度以及并发症发生情况。结果60例患者均顺利完成手术,术后3个月眼睑形态评分为(26.49±2.87)分,明显高于术前的眼睑形态评分(12.78±2.80)分(t=26.486,P<0.001);术后无重睑线凹陷过深52例、出现轻度重睑线凹陷过深8例,明显优于术前的重睑线凹陷过深分级为轻度18例、中度27例、重度15例(Z=-9.586,P<0.001);术后3个月患者满意度评分为(94.12±3.71)分,明显高于术前的患者满意度评分(54.96±5.64)分(t=44.933,P<0.001);术后3例患者出现上睑切口轻度增生,经药物治疗后恢复,其余患者均未出现切口增生、重睑线变浅、重睑过宽、重睑弧度不理想、上睑下垂、上睑眼窝凹陷及多层重睑等并发症。结论下睑眶隔脂肪移植修复术可有效改善重睑成形术后不良形态患者的上睑外观缺陷,且并发症较少,患者满意度较高,值得临床推广应用。

Objective To investigate the effect of fat grafting on orbital septum of lower eyelid in the treatment of poor morphology after double eyelid blepharoplasty.Methods From October 2019 to October 2021,60 patients with poor morphology after double eyelid blepharoplasty,admitted to Plastic Surgery Hospital of Weifang Medical University,received fat grafting on orbital septum of their lower eyelids.Eyelid morphology,classification of depression of double eyelid line,patients’satisfaction and occurrence of complications were observed.Results All the 60 patients went through the operation successfully.Three months after operation,the eyelid morphology score was(26.49±2.87)points,much higher than the(12.78±2.80)points before operation(t=26.486,P<0.001);after operation,52 cases didn’t have the problem of excessive depression of double eyelid line while 8 cases did,which was much better than that before operation where 18 cases suffered mild type of excessive depression of double eyelid line,27 cases moderate type,15 cases severe type(Z=-9.586,P<0.001);three months after operation,the patients’satisfaction score was(94.12±3.71)points,much higher than(54.96±5.64)points before operation(t=44.933,P<0.001);after operation,3 patients developed mild hyperplasia at upper eyelid incision,which recovered after drug treatment,whereas no other complications were observed in all the other patients including shallower double eyelid line,enlarged double eyelid,unsatisfactory double eyelid radian,ptosis,upper eyelid socket depression and multi-layer double eyelid,etc.Conclusion Fat grafting on orbital septum of lower eyelid can effectively improve the appearance defect of upper eyelid in patients with poor morphology after double eyelid blepharoplasty,cause few complications,and achieve much high patients’satisfaction.
