家庭主要照顾者的照顾反应对结直肠癌患者生活质量影响的初探

The study of the main family caregiver's care reaction on the quality of life of colorectal cancer patients

目的探讨家庭主要照顾者的照顾反应对结直肠癌患者生活质量的影响。方法选择2018年11月-2019年5月本院收治的296对结直肠癌患者及其家庭主要照顾者作为调查对象。采用一般资料调查表、领悟社会支持量表家庭支持维度、中文版简明健康SF-12量表对患者进行调查,同时对其家庭主要照顾者采用一般资料调查表、照顾者反应评估量表进行调查。数据采用独立样本t检验、单因素方差分析,Pearson相关性分析,多元线性回归分析等统计学方法。结果不同患者的民族、工作状态及家庭主要照顾者的文化程度比较,差异均具有统计学意义(P<0.05);照顾反应中的时间安排受打扰维度与患者生理健康及心理健康均有相关性(P<0.05);照顾反应中的家庭支持缺乏维度与患者心理健康有相关性(P<0.05);患者民族及照顾反应中的时间安排受打扰维度均进入生理健康影响因素的回归方程;患者民族、工作状态及照顾反应中的时间安排受打扰维度均进入心理健康影响因素的回归方程;分别解释结直肠癌患者生理健康及心理健康总变异的28.2%和22.6%。结论结直肠癌患者生活质量处于偏低水平,家庭缺乏支持会降低患者的心理健康,家庭主要照顾者的时间安排受打扰会降低患者的生活质量。医护人员应该以减轻家庭主要照顾者照顾负担为切入点促进患者生活质量,尤其是照顾者的时间安排和家庭支持;此外,还需更加关注无业及退休患者的心理健康,并实施针对性的干预措施以改善患者生活质量。

Objective To explore the impact of the main family caregiver's care response on the quality of life(QOL)of patients with colorectal cancer.Methods From December 2018to May 2019,296 colorectal cancer patients and their main family caregivers in the oncology department of our hospital were collected as respondent.The general information questionnaire,the family support dimension of perceived social support scale,and the Chinese version of the SF-12 Scale were used to carried out survey on patients;and general information questionnaire,caregiver reaction assessment scale were used to carried out investigation on the family caregivers.Independent sample t-test,one-way analysis of variance,Pearson correlation analysis,multiple linear regression analysis and other statistical methods are used to analyze the data.Results There were significant differences in ethnic group,working status and education level of main family caregivers among different patients(P<0.05).The disturbance dimension of the care reaction time arrangement(CRA)is correlated with the patient's physical health and mental health(P<0.05).The lack of family support dimension of care reaction is correlated with the mental health of the patient(P<0.05).Both the patient's nationality and disturbance dimension of the care reaction time arrangement enter the regression equation of physical health influencing factors.The patient's ethnicity,work status,and disturbance dimension of the care reactiontime arrangement all enter the regression equation of mental health influencing factors.The results explain separately 28.2%and 22.6%of the variation of physical health and mental health of patients.Conclusions The QOL of patients with colorectal cancer is at a low level.The lack of family support will reduce the patient's mental health,and the interruption of the family's main caregiver's schedule will reduce the patient's QOL.Medical staff should start to improve the QOL patients by reducing the burden of care for the main caregivers of the family,especially the caregivers'time arrangements and family support.In addition,more attention should be paid to the mental health of unemployed and retired patients,and implement targeted interventions to improve the QOL of patients.
