经皮椎间孔镜与开窗减压髓核摘除术治疗腰椎间盘突出的临床疗效分析

Analysis of the Clinical Effect of Percutaneous Transforaminal Endoscopy and Fenestration Decompression Nucleus Pulposus in the Treatment of Lumbar Disc Herniation

目的:对比分析在腰椎间盘突出患者的手术治疗中应用经皮椎间孔镜与开窗减压髓核摘除术的价值。方法:回顾性方式将我院2019年1月-2020年12月收治的80例腰椎间盘突出手术患者纳入研究,遵从“计算机随机分组法”将其分为对照组(38例,开窗减压髓核摘除术)和观察组(42例,经皮椎间孔镜术),观察2组患者围术期情况、治疗前后脊柱功能改善情况以及并发症(侧肢感觉迟钝、神经根损伤、腰椎不稳)发生率。结果:观察组患者手术耗时较长,但出血量较少,切口长度短且住院时间短;经手术治疗后患者脊柱功能改善情况较好,数据和对照组相比,P<0.05;观察组并发症发生率仅为2.3%,对照组为15.7%,P<0.05。结论:对腰椎间盘突出患者实施手术治疗,与开窗减压髓核摘除术相比,更建议推广经皮椎间孔镜髓核摘除术,可有效提高治疗效果并改善患者预后。

Objective:To compare and analyze the value of percutaneous transforaminal endoscopy and fenestration decompression nucleectomy in the surgical treatment of patients with lumbar intervertebral disc herniation.Methods:Retrospectively,80 patients with lumbar intervertebral disc herniation surgery in our hospital from 2019.1 to 2020.12 were included in the study,and they were divided into control group(38 cases,fenestrated decompression nucleus extraction)and observation group according to the"computer randomization method".Group(42 cases,percutaneous transforaminal endoscopy);the perioperative conditions,the improvement of spinal function before and after treatment,and the incidence of complications(lateral limb dysesthesia,nerve root injury,lumbar instability)were observed in the two groups.Results:The operation time of the observation group was longer,but the amount of blood loss was less,the length of the incision was shorter and the hospital stay was shorter;and the spinal function of the patients improved better after the operation,the data was compared with the control group,P<0.05.The incidence of complications was only 2.3%in the observation group and 15.7%in the control group,P<0.05.Conclusions:Compared with fenestrated decompression nucleus pulposus,it is recommended to promote percutaneous transforaminal endoscopic nucleus pulposus for surgical treatment of patients with lumbar intervertebral disc herniation,which can effectively improve the treatment effect and improve the prognosis of patients.
