非ST段抬高型急性冠脉综合征高敏肌钙蛋白I水平与SYNTAX评分的相关性分析

The correlation analysis between hs-cTnI level and SYNTAX score in patients with Non-ST-segment elevation acute coronary syndrome

目的探讨非ST段抬高型急性冠脉综合征(NSTE-ACS)患者的高敏肌钙蛋白I(hs-cTnI)水平与SYNTAX评分的相关性。方法选取2017年1月至2019年10月在中国医学科学院阜外医院ARCHITECT hs-cTnI多中心研究,临床确诊为NSTE-ACS并接受血运重建治疗的患者254例,患者年龄(60.5±10.1)岁,男性190例(74.8%)。计算每例患者的SYNTAX评分,将患者按照SYNTAX评分结果分为低分组(0~22分)、中分组(23~32分)和高分组(≥33分),三组间基线特征无统计学差异(P>0.05);检测患者就诊基线及就诊后1 h、3 h的hscTnI值,hs-cTnI的基线值。分析hs-cTnI水平与SYNTAX评分的相关性。结果在总体人群和男性人群中,hs-cTnI的基线值、1 h值、3 h值与SYNTAX评分正相关(P<0.05),在女性人群中,hs-cTnI的基线值、1 h值、3 h值与SYNTAX评分无相关性(P>0.05)。男性和女性的hscTnI基线值无统计学差异(P>0.05),而男性的hs-cTnI 1 h值和3 h值均显著高于女性(P<0.05)。结论NSTE-ACS患者hs-cTnI的基线值、1 h值、3 h值与SYNTAX评分正相关。

Objective To investigate the correlation between hs-cTnI level and SYNTAX score in patients with Non-ST-segment elevation acute coronary syndrome.Methods Patients who participated in the ARCHITECT hs-cTnI multi-center study of Fuwai Hospital from January 2017 to October 2019 and were clinically diagnosed with NSTE-ACS and received revascularization were enrolled.SYNTAX score of each patient was calculated.The correlation between hs-cTnI level and SYNTAX score was analyzed.Results The 254 NSTE-ACS patients had an average age of 60.5±10.1 years,and there were 190 males(74.8%).In the general population and male population,the baseline,1 h and 3 h values of hs-cTnI were positively correlated with SYNTAX score(P<0.05).There was no correlation between baseline,1 h and 3 h values of hs-cTnI and SYNTAX score in female population(P>0.05).Patients were divided into low score group(0-22),medium score group(23~32)and high score group(≥33)according to SYNTAX score.There were no statistical differences in baseline characteristics among the three groups,and no statistical differences in baseline,1 h and 3 h values of hs-cTnI.There was no significant difference in hs-cTnI baseline values between males and females(P>0.05),but the 1 h and 3 h values of hs-cTnI in males were significantly higher than those in females(P<0.05).Conclusion The baseline,1 h and 3 h values of hs-cTnI in NSTE-ACS patients were positively correlated with SYNTAX score.
