摘要
目的分析非肿块型乳腺癌在常规超声及自动乳腺全容积成像诊断系统(ABUS)的超声图像特征,探讨ABUS在非肿块型乳腺病变中的诊断价值。方法回顾性分析106例经病理确诊的非肿块型乳腺病变,由两位高年资医师分别对常规超声图像、ABUS联合常规超声图像进行阅片并给出BI-RADS分类,通过病理比较,分析两种方法的诊断准确性。结果106例非肿块型乳腺病变中,恶性病变60例(56.6%),非恶性病变46例(43.4%)。基于常规超声图像的诊断准确率为75.5%,基于常规超声图像结合ABUS的诊断准确率为87.7%,差异有统计学意义(P<0.05)。非肿块型乳腺癌在常规超声上多表现为伴或不伴微钙化的低回声区,病理显示恶性中以导管内癌居多。ABUS冠状面有助于提高对病变边缘及微钙化的显示。结论常规超声结合ABUS对提高非肿块型乳腺癌的诊断准确率有积极意义。
Objective To analyze the ultrasound characteristics of non-mass breast cancers and explore the value of automated breast ultrasound system(ABUS)in the diagnosis of non-mass breast cancers.Methods This retrospective study involved 106 non-mass breast lesions which were confirmed by pathology.All breast lesions were examined by hand-held ultrasound(HHUS)and ABUS.The images were reviewed by 2 experienced readers and the Breast Imaging Reporting and Data System(BI-RADS)was used to interpret.The diagnostic value of the two methods was compared.Results Among the 106 non-mass breast lesions,60 cases(56.6%)were malignant and 46 cases(43.4%)were benign.The diagnostic accuracy rate of ABUS combined with HHUS group was 87.7%,and that of HHUS group was 75.5%,the difference was statistically significant(P<0.05).Non-mass breast cancers often appear as hypoechoic areas whatever in HHUS or ABUS.The pathological results show that ductal carcinoma in situ is the most common malignant.Conclusion ABUS helps to improve the display of micro-calcification and the detection rate of non-mass breast cancers.
