经内镜逆行性胰胆管造影术中空气栓塞抢救成功1例

Successful diagnosis and treatment of air embolism in endoscopic retrograde cholangiopancreatography:a case report

摘要 患者,男性,年龄31岁,身高174 cm,体重70 kg,因"胆管结石、先天性胆总管囊肿、胆囊切除术后状态",拟在全身麻醉下行经内镜逆行性胰胆管造影术(ERCP)。既往手术史:患者9岁时行"先天性胆总管囊肿切除+胆总管空肠ROX-Y吻合术"。2016年行"残余胰腺段胆总管囊肿切除+复杂肠粘连松解+胆道探查取石+空肠造瘘+T管引流术"。2022年5月9日尝试行小肠镜经吻合口取石,检查中发现吻合口5 mm,右肝管呈"上窄下宽"型狭窄,故中断操作。入院时ASA分级Ⅱ级,心功能分级Ⅰ级。2022年5月26日胸部CT结果示:右侧胸膜稍增厚;肝内、外胆管积气,肝门部高密度结节,胆管结石。
