16层螺旋CT对急性阑尾炎的诊断价值探讨

Diagnostic Value Exploration of 16 Slice Spiral CT for Acute Appendicitis

目的研究急性阑尾炎应用16层螺旋CT诊断的价值。方法研究对象选取本院于2020年1~10月收治的103例急性阑尾炎患者,均采用飞利浦MX16-slice型16排螺旋CT机行16排螺旋CT检查,记录、分析CT扫描结果,以病理诊断结果作为金标准,将CT检查结果与病理诊断结果进行比较,计算CT对阑尾结构、急性阑尾炎类型的诊断符合率,并分析不同类型急性阑尾炎的CT影像学表现。结果16层螺旋CT对阑尾结构完整可辨诊断符合率为95.65%,对阑尾结构不清诊断符合率为100.00%,对阑尾结构难辨诊断符合率为92.00%,对团块诊断符合率为100.00%;总诊断符合率比较,16层螺旋CT与病理诊断结果并无明显差异(P>0.05);16层螺旋CT对单纯性急性阑尾炎诊断符合率为100.00%,对化脓性急性阑尾炎诊断符合率为76.19%,对坏疽性急性阑尾炎诊断符合率为100.00%;总诊断符合率比较,16层螺旋CT对单纯性、化脓性及坏疽性急性阑尾炎诊断结果与病理诊断结果并无明显差异(P>0.05)。结论急性阑尾炎应用16层螺旋CT的诊断准确率较高,可为临床治疗提供可靠依据。

Objective To study diagnostic value of 16 slice spiral CT for acute appendicitis.Methods The paper chose 103 patients with acute appendicitis admitted to our hospital from January 2020 to October 2020 as study subjects,examined with Philips MX16 slice 16 row spiral CT machine.CT scanning results were recorded and analyzed with gold standard of pathological diagnosis results.CT results was compared with pathological diagnosis results,and diagnostic coincidence rate of CT on structure of appendix and type of acute appendicitis was calculated,CT findings of different types of acute appendicitis were analyzed.Results Diagnostic accuracy of 16 slice spiral CT was 95.65%for complete appendix structure,100.00%for unclear appendix structure,92.00%for difficult appendix structure and 100.00%for mass,with no significant difference between the results of 16 slice spiral CT and pathological diagnosis(P>0.05);diagnostic accuracy of 16 slice spiral CT was 100.00%for simple acute appendicitis,76.19%for suppurative acute appendicitis,and 100.00%for gangrenous acute appendicitis,with no significant difference between results of 16 slice spiral CT and pathological diagnosis of simple,purulent and gangrenous acute appendicitis(P>0.05).Conclusion 16 slice spiral CT can achieve high diagnostic accuracy for acute appendicitis,and provide reliable basis for clinical treatment.
