摘要
目的 探讨xSPECT/CT断层骨显像在小儿神经母细胞瘤骨转移诊断中的应用价值。方法 回顾性分析经病理证实的108例神经母细胞瘤患儿的全身骨显像、xSPECT/CT及SPECT/CT断层骨显像资料,比较xSPECT/CT断层骨显像与SPECT/CT断层骨显像两种不同图像对神经母细胞瘤骨转移诊断的敏感度、特异度、准确率、阳性预测值、阴性预测值、核医学医师的诊断信心及图像质量。结果 xSPECT/CT断层骨显像对小儿神经母细胞瘤骨转移诊断的敏感度、特异度、准确率、阳性预测值、阴性预测值分别为95.24%、95.56%、95.37%、96.77%、93.48%;SPECT/CT断层骨显像的敏感度、特异度、准确率、阳性预测值、阴性预测值分别为84.13%、95.56%、88.89%、96.36%、81.13%,且xSPECT/CT断层骨显像能提高核医学医师的诊断信心及图像质量,差异均具有统计学意义(P<0.05)。结论 xSPECT/CT能提高图像质量,同时提升核医学医师对骨转移的诊断信心和诊断质量,从而早期诊断患儿骨转移情况,进一步改善患儿预后。
Objective To investigate the value of xSPECT/CT in the diagnosis of bone metastases of pediatric neuroblastoma. Methods Whole body bone imaging, xSPECT/CT and SPECT/CT tomographic bone imaging data of 108 children with neuroblastoma confirmed by pathology were retrospectively analyzed.The sensitivity, specificity, accuracy, positive predictive value, negative predictive value, diagnostic confidence of nuclear medicine physicians, and image quality of the two different images for the diagnosis of bone metastases of neuroblastoma with xSPECT/CT and SPECT/CT tomographic bone imaging were compared. Results The sensitivity, specificity, accuracy, positive predictive value, and negative predictive value of xSPECT/CT tomographic bone imaging for the diagnosis of bone metastasis of pediatric neuroblastoma were 95.24%,95.56%,95.37%,96.77%,and 93.48%,respectively;the sensitivity, specificity, accuracy, positive predictive value, and negative predictive value of SPECT/CT tomographic bone imaging were 84.13%,95.56%,88.89%,96.36%,81.13%,and xSPECT/CT tomographic bone imaging could improve the diagnostic confidence and image quality of nuclear medicine physicians, and the differences were statistically significant(P<0.05). Conclusion xSPECT/CT can improve the image quality, and at the same time enhance the confidence and quality of the diagnosis of bone metastases by nuclear medicine physicians to diagnose bone metastases in children early and further improve the prognosis of children.
