基于关联规则和聚类分析的附子半夏配伍治疗恶性肿瘤用药规律研究

Study on the rule of compatibility of radix aconiti and rhizoma pinelliae in the treatment of malignant tumors based on association rules and cluster analysis

目的:运用关联规则和聚类分析等数据挖掘技术,总结附子半夏临床配伍治疗恶性肿瘤的用药规律,为附子半夏同方配伍治疗恶性肿瘤提供科学依据与数据参考。方法:从我院HIS系统中,导出2015年1月至2021年8月附子半夏同方配伍的处方资料,对处方中附子半夏的用量、比例、临床诊断、高频中药进行统计分析,对配伍药物进行关联规则分析、系统聚类分析。结果:入选317张处方中,附子与半夏的常用配伍比例为1∶1,配伍药物频数最多的为温中药及补气药。基于关联规则分析,挖掘出高关联性药对组合15种,常用药对组合有炙甘草-土鳖虫、全蝎-蜈蚣-熟地黄、全蝎-蜈蚣-党参等;基于系统聚类分析,发现4个有效聚类群,得到常用方附子理中汤、潜阳封髓丹。结论:附子半夏同方配伍在恶性肿瘤的治疗中比较多见,中药配伍规律显示临床总治疗原则以温阳散寒、化痰散结为主,这些结论可为附子半夏的临床安全合理应用提供数据参考。

Objective:To summarize the clinical compatibility of Radix Aconiti and Pinelliae in the treatment of malignant tumors by using data mining techniques such as association rules and cluster analysis,so as to provide scientific basis and data reference for the compatibility of Radix Aconiti and Pinelliae in the treatment of malignant tumors.Methods:From the HIS system of our hospital,the prescription data of the compatibility of Radix Aconiti and Pinelliae from January 2015 to August 2021 were derived,the dosage,proportion,clinical diagnosis and high-frequency Chinese medicine in the prescription were statistically analyzed,and the association rule analysis and systematic cluster analysis were performed on the compatible drugs.Results:Among 317 prescriptions selected,the commonly used compatibility ratio of aconite and pinellia ternata was 1:1,and the most frequently used compatibility drugs were warm herbs and qi tonifying drugs.Based on the analysis of association rules,15 kinds of high association drug pair combinations were discovered,and the commonly used drug pair combinations were red licorice ground beetle,scorpion centipede rehmannia,scorpion centipede codonopsis,etc;Based on the systematic cluster analysis,four effective clusters were found,and the commonly used formula Fuzi Lizhong Decoction and Qianyang Fengsui Pill were obtained.Conclusion:The compatibility of aconite and pinellia ternata with the same formula is more common in the treatment of malignant tumors.The rule of compatibility of traditional Chinese medicine shows that the general clinical treatment principle is mainly to warm yang and dissipate cold,and dissipate phlegm and disperse nodule.
