Mr.ZHU Guiting’s experience in the differentiation and treatment of chronic atrophic gastritis

慢性萎缩性胃炎是以胃黏膜上皮及腺体萎缩退化为病理表现的胃炎。西医对其病因和发病机制并没有完全明确,普遍认为与幽门螺杆菌感染、胆汁反流、免疫失调有关。中医认为慢性萎缩性胃炎与情志失调、饮食不节、药物、外邪等多种因素损伤脾胃,脾胃气虚和肝脾密切相关,在治疗上有一定优势。祝桂庭是山东省名老中医,他注重证候辨证与微观辨证,根据该病病机临床以脾胃虚弱、肝郁气滞、脾胃湿热为主,分别予香砂六君子汤、柴胡疏肝散、半夏泻心汤加减参以现代中药药理研究的应用,重视饮食、心理调护,在中医治疗萎缩性胃炎方面有丰富的经验。文章总结了其临床经验,为临床辨证治疗萎缩性胃炎提供更多参考。

Chronic atrophic gastritis is a gastritis with gastric mucosal epithelial atrophy and gland degeneration into pathological manifestations.The etiology and pathogenesis in Western medicine is not completely clear,and it is generally believed to be related to helicobacter pylori infection,bile reflux,and immune imbalance.In traditional Chinese medicine,chronic atrophic gastritis is closely related to a variety of damages to the spleen and stomach by emotional disorders,diet disorders,drugs,external evil and other factors,resulting in the spleen and stomach Qi(气)deficiency.TCM shows certain advantages in the treatment.Mr.ZHU Guiting is a famous veteran TCM practitioner in Shandong Province.He paid attention to syndrome differentiation and microcosmic syndrome differentiation.According to the pathogenesis of the disease,the clinical treatment of the disease mainly focused on spleen and stomach weakness,liver depression and Qi stagnation,and spleen and stomach damp-heat.And the Xiangsha Liujunzi decoction(香砂六君子汤),Chaihu Shugan San(柴胡疏肝散),the Banxia Xiexin decoction(半夏泻心汤)were applied respectively.On the basis of modern TCM medicine pharmacological research,he also pays attention to diet and psychological care,and has rich experience in the treatment of atrophic gastritis in TCM.In the article,his rich clinical experience is summarized to provide more reference for clinical treatment of the disease based on syndrome differentiation.
