布地奈德雾化吸入对老年慢性阻塞性肺气肿加重期患者血气分析、肺功能及炎性因子的影响 被引量:3

摘要 目的研究布地奈德雾化吸入对老年慢性阻塞性肺气肿加重期患者血气分析、肺功能及炎性因子的影响。方法选取2018年12月—2021年12月就诊的121例老年慢性阻塞性肺气肿加重期,根据治疗方法分为研究组63例和对照组58例。研究组在常规治疗基础上予以布地奈德雾化吸入,对照组予以常规治疗。比较2组临床疗效、血气分析指标[动脉血氧分压(PaO_(2))、二氧化碳分压(PaCO_(2))]、肺功能[用力肺活量(FVC)、第1秒用力呼气容积(FEV1)、最大呼气峰流速(PEF)]、炎性因子[C反应蛋白(CRP)、白细胞介素-6(IL-6)、肿瘤坏死因子-α(TNF-α)]以及不良反应发生情况。结果研究组临床总有效率高于对照组(P<0.05)。治疗后,2组CRP、IL-6、TNF-α水平及动脉血PaCO_(2)较治疗前降低,FVC、FEV1、PEF及动脉血PaO_(2)较治疗前升高,且研究组CRP、IL-6、TNF-α水平及动脉血PaCO_(2)低于对照组,FVC、FEV1、PEF及动脉血PaO_(2)高于对照组(P<0.05,P<0.01)。2组不良反应总发生率比较差异无统计学意义(P>0.05)。结论对老年慢性阻塞性肺气肿加重期患者采取布地奈德雾化吸入治疗效果确切,可有效改善患者血气分析、肺功能指标,减轻炎症反应,且安全性尚可。 Objective To investigate the effects of Budesonide aerosol inhalation on blood gas analysis,pulmonary function and inflammatory factors in elderly patients with exacerbation of chronic obstructive pulmonary emphysema.Methods A total of 121 elderly patients with exacerbation of chronic obstructive emphysema from December 2018 to December 2021 were selected and divided into the research group(n=63)and the control group(n=58)according to different treatment methods.The research group was given Budesonide aerosol inhalation on the basis of conventional treatment,while the control group was given conventional treatment.The clinical efficacy,blood gas analysis indexes[partial pressure of arterial oxygen(PaO_(2)),partial pressure of carbon dioxide(PaCO_(2))],pulmonary function[forced vital capacity(FVC),forced expiratory volume in the first second(FEV1),maximum peak expiratory flow rate(PEF)],inflammatory factors[C reactive protein(CRP),interleukin-6(IL-6),tumor necrosis factor-α(TNF-α)]and the occurrence of adverse reactions were compared between the two groups.Results The total clinical effective rate of the research group was higher than that of the control group(P<0.05).After treatment,the levels of CRP,IL-6,TNF-αand arterial blood PaCO_(2)in the two groups were reduced compared with those before treatment,while FVC,FEV1,PEF and arterial blood PaO_(2)were increased compared with those before treatment;CRP,IL-6,TNF-αand arterial blood PaCO_(2)in the research group were lower than those in the control group,while FVC,FEV1,PEF and arterial PaO_(2)were higher than those in control group(P<0.05,P<0.01).There was no significant difference in the total incidence of adverse reactions between the two groups(P>0.05).Conclusion Budesonide atomization inhalation is effective in the treatment of elderly patients with exacerbation of chronic obstructive emphysema,which can effectively improve blood gas analysis and pulmonary function indicators,and reduce inflammation,and the safety is reasonable.
