温度照射保温计划配合预见性护理对断指合并软组织缺损再植患者术后功能恢复的影响

Effects of Temperature Irradiation and Thermal Insulation Plan Combined with Predictive Nursing on Postoperative Functional Recovery of Patients with Amputated Finger and Soft Tissue Defect Replantation

目的:探讨温度照射保温计划配合预见性护理对断指合并软组织缺损再植患者术后功能恢复的影响。方法:选取2018年3月—2020年5月郑州大学附属郑州中心医院收治的115例断指合并软组织缺损再植患者作为研究对象,根据分层区组随机法分为对照组57例和观察组58例。对照组采用常规护理,观察组采用温度照射保温计划配合预见性护理。比较两组患者对外观满意度、指关节活动度、握力、肿胀率以及指功能恢复情况。结果:护理后2个月,观察组握力和指关节活动度大于对照组,差异有统计学意义(t=22.346、8.456,P<0.05)。护理后2个月后,两组患者断指再植存活率比较,差异无统计学意义(χ^(2)=2.071,P>0.05);观察组有知觉、可以活动高于对照组,无知觉低于对照组,差异有统计学意义(χ^(2)=4.044、1.014、3.898,P<0.05)。两组患者肿胀率比较,观察组肿胀率低于对照组,差异有统计学意义(χ^(2)=17.092,P<0.05)。观察组护理后2周、护理后1个月、护理后2个月对皮瓣外观美感满意率高于对照组,差异有统计学意义(χ^(2)=7.653,P=0.011)。结论:对断指合并软组织缺损再植患者实施温度照射保温计划配合预见性护理,能够促使指关节功能恢复,降低肿胀率。

Objective:To explore the effects of temperature irradiation and thermal insulation plan combined with predictive nursing on postoperative functional recovery of patients with amputation combined with soft tissue defect replantation.Methods:A total of 115 patients with amputated fingers combined with soft tissue defect replantation who were admitted to the hospital from March 2018 to May 2020 were selected as the research objects,and divided into the control group of 57 cases and the observation group of 58 cases according to the stratified block random method.The control group received routine nursing care,and the obser⁃vation group received temperature irradiation and heat preservation plan combined with predictive nursing.The appearance satisfac⁃tion,range of motion of finger joints,grip strength,swelling rate and recovery of finger function were compared between the two groups.Results:2 months after the nursing,and the observation group was greater than the control group,and the difference was statistically significant(t=22.346,8.456,P<0.05).2 months after nursing,there was no significant difference in the survival rate of severed finger replantation between the two groups(χ^(2)=2.071,P>0.05).The rate of perception and activity in the observation group was higher than that in the control group,and the rate of non-perception was lower than that in the control group,and the differences were statistically significant(χ^(2)=4.044,1.014,3.898,P<0.05).The swelling rate in the observation group was lower than that in the control group,and the difference was statistically significant(χ^(2)=17.092,P<0.05).The aesthetic satisfaction rates of flaps in the observation group were after 2 weeks,1 month,and 2 months after nursing,which were higher than in the control group,the difference was statistically significant(χ^(2)=7.653,P=0.011).Conclusion:For patients with severed fingers combined with soft tissue defect replantation,temperature irradiation and heat preservation plan combined with predictive nursing can promote the recovery of finger joint function and reduce the swelling rate.
