调中健脾汤联合米曲菌胰酶片对功能性消化不良患者胃肠激素及细胞因子水平的影响

Effects of Tiaozhong Jianpi Decoction combined with oryz-aspergillus enzyme and pancreatin tablets on gastrointestinal hormones and cytokines in patients with functional dyspepsia

目的:观察调中健脾汤联合米曲菌胰酶片对功能性消化不良(FD)患者胃肠激素及细胞因子水平的改善情况。方法:选取2019年1月—2021年1月就诊的120例FD患者为研究对象,按照完全随机分组法将120例患者分为研究组和对照组,各60例。两组患者均接受一般治疗,对照组服用米曲菌胰酶片,研究组服用米曲菌胰酶片+调中健脾汤,两组均治疗1个月,并于治疗结束后接受为期6个月的随访。评估两组患者治疗1个月时的疗效,主要观察指标包括治疗前、治疗4周时胃肠激素[胃泌素(GAS)、胃动素(MTL)、生长抑素(SS)]、炎症因子[IL-1β、IL-6、肿瘤坏死因子-α(TNF)-α]及随访期间的复发率。结果:在经过1个月的治疗后,研究组患者的治疗总有效率与对照组相比差异无统计学意义(P>0.05);两组患者的血清GAS、SS水平相较于治疗前降低,MTL水平相较于治疗前升高,血清IL-1β、IL-6、TNF-α水平相较于治疗前降低,且研究组各指标的改善情况相较于对照组更加显著,两组差异有统计学意义(均P<0.05)。两组患者随访1个月、3个月时的复发率相比较,差异无统计学意义(P>0.05);在随访第3~6个月时,研究组的复发率显著低于对照组,差异有统计学意义(P<0.05)。结论:FD患者接受调中健脾汤联合米曲菌胰酶片治疗后疗效显著,患者胃肠激素水平和细胞因子水平均得到了良好改善,有利于降低复发风险。

Objective:To observe the improvement of gastrointestinal hormones and inflammatory factors in patients with functional dyspepsia(FD)treated with Tiaozhong Jianpi Decoction combined with oryz-aspergillus enzyme and pancreatin tablets.Methods:One hundred and twenty patients with FD admitted to the hospital from January 2019 to January 2021 were selected for the study.According to the complete random grouping method,120 patients were equally divided into study group and control group,with 60 cases in each group.According to the consensus of experts,the patients in both groups received general treatment.At the same time,the control group took oryz-aspergillus enzyme and pancreatin tablets and the study group took oryz-aspergillus enzyme and pancreatin tablets+Tiaozhong Jianpi Decoction.Both groups were treated for 4 weeks and were followed up for 6 months after treatment.The efficacy of the two groups at 4 weeks of treatment was evaluated,and the main observation indexes included gastrointestinal hormones before and 4 weeks after treatment[gastrin(GAS),motilin(MTL),somatostatin(SS)],inflammatory factors[interleukin(IL)-1β,IL-6,tumor necrosis factor-α(TNF)-α]and the recurrence rate during follow-up.Results:After 4 weeks of treatment,there was no statistical significant difference in the total effective rate between the study group and the control group(P>0.05).After four weeks of treatment,compared with before treatment,serum GAS and SS were decreased,MTL was increased,serum IL-1β,IL-6 and TNF-αwere decreased of patients in both groups,and the improvement of indexes in the study group was more significant than that in the control group.The difference between the two groups was statistically significant(P<0.05).There was no statistical significant difference in the recurrence rate between the two groups at 1 month and 3 months of follow-up(P<0.05);the recurrence rate of the study group was significantly lower than that of the control group from the 3 rd month to the 6 th month of follow-up,the difference was statistically significant(P<0.05).Conclusion:Patients with FD after Tiaozhong Jianpi Decoction combined with oryz-aspergillus enzyme and pancreatin tablets treatment obtain significant curative effect.Gastrointestinal hormones and inflammatory factors in patients with FD are improved,which is conducive to reducing the risk of symptom recurrence.
