两种麻醉药物在老年患者全身麻醉术中的应用效果及对其应激反应的影响比较分析

Comparative Analysis of the Effects of Two Anesthetic Drugs in General Anesthesia and Their Effects on Stress Response in Elderly Patients

目的探讨两种麻醉药物在老年患者全身麻醉术中的应用效果及对其应激反应的影响。方法选取茂名市中医院2020年11月-2021年11月收治的需要进行全身麻醉手术的老年患者106例,采用随机数字表法将患者分为两组,每组53例。观察组患者给予瑞芬太尼麻醉,对照组患者给予芬太尼麻醉。比较两组患者各相关指标。结果观察组患者的麻醉起效时间、睁眼时间、术后镇痛药物使用时间、自主呼吸恢复时间及拔管时间均短于对照组(P<0.05)。麻醉诱导前,两组患者的SBP、DBP、HR指标无显著差异(P>0.05);插管即刻、插管后1min,观察组患者的SBP、DBP、HR指标均高于对照组(P<0.05)。观察组患者呼吸抑制、躁动、呛咳及心血管事件应激反应发生率均低于对照组(P<0.05)。结论老年患者全身麻醉术中麻醉药物瑞芬太尼可有效缓和患者的各项相关指标,麻醉质量较高,促进患者早期苏醒,临床应激反应相对较低,疗效显著,值得临床推广应用。

Objective To investigate the effect of two kinds of anesthetic drugs on the stress response of elderly patients under general anesthesia.Methods 106 elderly patients who need general anesthesia in our hospital from November 2020 to November 2021 were selected and randomly divided into two groups,53 cases in each group.Patients in observation group were given remifentanil anesthesia,while patients in control group were given fentanyl anesthesia.The related indexes of the two groups were compared.Results The onset time of anesthesia,eye opening time,use time of postoperative analgesics,recovery time of spontaneous respiration and extubation time in the observation group were shorter than those in the control group(P<0.05).Before the anesthesia induction,there was no significant difference in SBP,DBP,and HR indicators in the two groups(P>0.05);the indexes of SBP,DBP and HR in the observation group were higher than those in the control group immediately after intubation and 1 min after intubation(P<0.05).The incidence of respiratory depression,agitation,choking and stress response to cardiovascular events in the observation group were lower than those in the control group(P<0.05).Conclusion During general anesthesia for elderly patients,the anesthetic drug rifentanil can effectively alleviate various relevant indexes of patients,improve the quality of anesthesia,promote the early awakening of patients,relatively low clinical stress response and significant curative effect,which is worthy of clinical application.
