两种不同手术方式治疗肩袖损伤的临床效果对比

Comparison of Clinical Effects of Two Different Surgical Methods for Rotator Cuff Injury

目的比较全肩关节镜、关节镜下小切口不同手术方式治疗肩袖损伤的临床效果。方法于2017年8月-2020年12月选取本院收治的81例肩袖损伤患者作为研究对象,随机分为研究组(全肩关节镜手术,n=40)、对照组(关节镜下小切口手术,n=41),术后随访6个月,比较组间术中出血量、手术时间、住院时间,比较不同时间点肩关节疼痛(VAS)评分、肩关节功能(UCLA)评分,以及术后并发症情况。结果研究组患者手术时间长于对照组,但术中出血量少于对照组,住院时间短于对照组,差异有统计学意义(P<0.05);术后随访1、3、6个月不同时间点,研究组、对照组VAS、UCLA评分均优于术前(P<0.05),但两组间比较,差异无统计学意义(P>0.05)。两组患者术后随访期间并发症发生率比较,差异无统计学意义(P>0.05)。结论全肩关节镜、关节镜小切口手术方式均能减轻肩袖损伤患者疼痛、改善关节功能,但全肩关节镜术中出血更少、恢复时间更短,而关节镜小切口手术时间更短。

Objective To compare the clinical effects of different surgical methods for rotator cuff injury under total arthroscopy and arthroscopy.Methods From August 2017 to December 2020,81 patients with rotator cuff injury admitted to our department were selected as the study subjects and randomly divided into the study group(total shoulder arthroscopic surgery,n=40)and the control group(small incision surgery under arthroscopy,n=41).The patients were followed up for 6 months after operation.The intraoperative bleeding volume,operation time and hospitalization time between the groups were compared,and the shoulder pain(VAS)score and shoulder function(UCLA)score at different time points were compared,And postoperative complications.Results The operation time of the patients in the study group was longer than that in the control group,but the intraoperative bleeding volume was less than that in the control group,and the hospitalization time was shorter than that in the control group.The difference was statistically significant(P<0.05);The vas and UCLA scores of the study group and the control group were better than those before operation at different time points of follow-up of 1,3 and 6 months after operation(P<0.05),but there was no difference between the two groups,and the difference was not statistically significant(P>0.05).There was no significant difference in the complication rate between the two groups during the follow-up period(P>0.05).Conclusion Both total shoulder arthroscopy and arthroscopic small incision surgery can reduce pain and improve joint function in patients with rotator cuff injury,but total shoulder arthroscopy has less bleeding and shorter recovery time,and arthroscopic small incision surgery has shorter time.
