前列腺多参数磁共振评估肿瘤负荷与临床分期的研究

A study of tumor burden and clinical staging assessed by multiparametric magnetic resonance imaging of the prostate

目的研究前列腺多参数磁共振(mpMRI)评估肿瘤负荷与临床分期的价值。方法92例前列腺癌患者均行mpMRI检查:T2加权成像(T2WI)、弥散加权成像(DWI)动态增强成像(DCE-MRI),对数据进行统计分析。结果T2WI评分与阳性针数、肿瘤占比、Gleason评分、分期呈正相关(P<0.05);DWI评分与PSA、阳性针数、肿瘤占比、Gleason评分呈正相关(P<0.05);术前mpMRI检查与术后分期变化无相关性(P>0.05)。结论T2WI可评估肿瘤负荷、病理组织学分级、肿瘤分期,DWI、DCE-MRI在评估肿瘤分期、肿瘤负荷中的价值有限,三种模式联合可为临床决策提供更多、更有效、更准确的参考信息。

Objective To investigate the value of prostate multiparametric magnetic resonance imaging(mpMRI)in assessing tumor load and clinical staging.Methods 92 patients with prostate cancer were examined by mpMRI:T2-weighted imaging(T2WI),diffusion-weighted imaging(DWI)and dynamic enhanced imaging(DCE-MRI),and the data were statistically analyzed.Results T2WI score was positively correlated with the number of positive stitches,tumor percentage,Gleason score,and staging(P<0.05);DWI score was positively correlated with PSA,number of positive stitches,tumor percentage,and Gleason score(P<0.05);preoperative mpMRI examination was not correlated with postoperative staging changes(P>0.05).Conclusions T2WI can assess tumor load,pathological histological grading and tumor staging,while DWI and DCE-MRI have limited value in assessing tumor staging and tumor load,respectively.The combination of the three modalities can provide more,more effective and more accurate reference information for clinical decision-making.
