摘要 目的 探讨高通量透析治疗对尿毒症血液透析患者微炎症及营养不良的影响。方法 选取漳州市第三医院肾内科2020年1-11月110例尿毒症血液透析患者,并在期间给予组别划分。其中的对照组应用常规血液透析,观察组使用高通量透析。对比两组的肾功能及相关指标、微炎症指标、营养指标。结果 两组血肌酐(serum creatinine,Scr)、β_(2)微球蛋白(β_(2)-microglobulin,β_(2)-MG)、总胆固醇(total cholesterol,TC)、三酰甘油(Triglyceride,TG)、超敏C-反应蛋白(High sensitivity C-reactive protein,hs-CRP),肿瘤坏死因子-α (tumor necrosis factor-α,TNF-α,TNF-α)、白介素-6(interleukin-6,IL-6)、总蛋白(total protein,TP)、白蛋白(albumin,Alb)、血红蛋白(hemoglobin,Hb)水平相比差异无统计学意义(P>0.05)。治疗后,与对照组比较,观察组Scr、β_(2)-MG、TC、TG水平较低(P<0.05);经治疗后观察组hs-CRP、TNF-α、IL-6水平低于对照组,差异有统计学意义(P<0.05);观察组治疗后TP、Alb、Hb水平明显高于对照组(P<0.05)。结论 高通量透析治疗能有效清除各个分子量毒素,改善机体内环境,明显减轻尿毒症血液透析患者微炎症状态,改善营养状态,保护肾功能,具有积极的临床意义。 Objective To investigate the effect of high-throughput dialysis treatment on microinflammation and malnutrition in uremic hemodialysis patients.Methods A total of 110 cases of uremia hemodialysis patients were selected from January 2020to November 2020 of Department of Nephrology,Zhangzhou Third Hospital and during the group division,the control group using routine hemodialysis,observation group using highthroughput dialysis,compare the two groups of kidney function and related indicators,micro inflammation indicators,nutritional indicators.Results There was no significant difference in the levels of Scr,β_(2)-MG,TC,TG,hs-CRP,TNF-α,IL-6,TP,Alb and H between the two groups before treatment (P>0.05).After treatment,compared with the control group,the levels of Scr,β_(2)-MG,TC and TG in the observation group were lower (P<0.05).After treatment,the levels of hs-CRP,TNF-α and IL-6in the observation group were significantly lower than those in the control group (P<0.05).After treatment,the levels of TP,Alb and Hb in observation group were significantly higher than those in control group (P<0.05).Conclusion Highthroughput dialysis treatment can effectively remove each molecular weight toxin,improve the internal environment,significantly reduce the microinflammation status,improve the nutritional status,and protect the renal function,with positive clinical significance.
