鼓室内注射地塞米松对梅尼埃病患者的治疗效果观察

Therapeutic Effect of Intratympanic Injection of Dexamethasone on Meniere’s Disease

目的 观察鼓室内注射地塞米松对梅尼埃病患者的疗效、听阈值及平衡功能的影响分析。方法 选取广州医科大学附属市八医院2018年1月—2021年1月收治梅尼埃病患者86例,随机分为两组,各43例。对照组实施5%碳酸氢钠静脉注射等基本常规药物治疗,研究组在对照组的基础上配合鼓室内注射地塞米松。比较两组患者在治疗前、后的眩晕障碍评分(dizzinesshandicapinventory,DHI)、听阈值、平衡功能、自我护理能力、生活质量,并判定治疗效果,统计患者对治疗的满意率。结果 两组患者治疗前的眩晕障碍评分、听阈值、平衡功能评分、自我护理能力评分相比,差异无统计学意义(P> 0.05)。治疗后两组患者平衡功能、自我护理能力均较治疗前提升,研究组高于对照组,且治疗后两组患者听阈值、眩晕障碍评分较治疗前下降,研究组低于对照组,差异有统计学意义(P <0.05)。治疗后两组生活质量评分较治疗前提升,研究组高于对照组,差异有统计学意义(P<0.05)。经治研究组患者的治疗总有效率与对治疗的满意率均高于对照组,差异有统计学意义(P <0.05)。结论 梅尼埃病患者以鼓室内注射地塞米松治疗,可明显的减轻患者的眩晕障碍,改善平衡功能、自护能力与生活质量,获得较高的治疗有效率,患者满意度高,效果理想。

Objective To observe the effect of intratympanic injection of dexamethasone on Meniere’s disease,auditory threshold and balance function.Methods A total of 86patients with Meniere’s disease treated in the Eighth Affiliated Hospital of Guangzhou Medical University from January2018 to January 2021 were randomly divided into two groups with 43 cases in each group.The control group was treated with intravenous injection of 5% sodium bicarbonate,while the research group was treated with intratympanic injection of dexamethasone on the basis of the control group.Dizziness inventory(DHI), hearing threshold,balance function,self-care ability and quality of life were compared between the two groups before and after treatment,and the treatment effect was judged,and the satisfaction rate of patients with treatment was counted.Results There was no significant difference in the scores of vertigo disorder,hearing threshold,balance function and self-care ability between the two groups before treatment(P> 0.05).After treatment,the hearing threshold,balance function and self-care ability of the two groups were improved compared with those before treatment,and the score of vertigo disorder in the study group was lower than that in the control group(P <0.05).There was no significant difference in quality of life score between the two groups before treatment(P> 0.05).After treatment,the quality of life scores of the two groups were higher than before treatment,and the study group was higher than the control group,with significant difference(P <0.05).After treatment,the total effective rate and satisfaction rate of the study group were higher than those of the control group,the difference was significant(P <0.05).Conclusion The patients with Meniere’ s disease,intratympanic injection of dexamethasone can significantly reduce vertigo disorder,improve balance function,self-care ability and quality of life,obtain higher treatment efficiency,high patient satisfaction and ideal effect.
