摘要
目的观察不同T、M分期前列腺癌小视野体素内不相干运动(IVIM)弥散加权成像(DWI)参数特征。方法回顾性分析46例接受前列腺MR常规序列成像和小视野IVIM-DWI的不同临床T、M分期及Gleason评分前列腺癌患者,观察其间标准表观弥散系数(ADC_(standard))、扩散系数(D)、伪扩散系数(D)及快速扩散分数(f)差异,分析临床T、M分期及Gleason评分与上述参数的相关性。结果不同T分期前列腺癌ADC_(standard)、D及f差异具有统计学意义(P均<0.05);其中,ADC_(standard)和f在T2与T3期、T2与T4期之间差异均有统计学意义(P均<0.05),D在T2与T4期、T3与T4期之间差异均有统计学意义(P均<0.05)。不同M分期及不同Gleason评分前列腺癌ADC_(standard)和D差异均有统计学意义(P均<0.05)。前列腺癌T分期与ADC_(standard)、D及f呈负相关(r=-0.69、-0.75、-0.53,P均<0.05),M分期和Gleason评分均与ADC_(standard)(r=-0.34、-0.50,P均<0.05)及D(r=-0.43、-0.53,P均<0.05)呈负相关。结论不同T分期前列腺癌ADC_(standard)、D及f存在差异,且T分期与三者均呈负相关;不同M分期前列腺癌ADC_(standard)和D存在差异,且M分期与二者均呈负相关。
Objective To observe the characteristics of reduced field-of-view intravoxel incoherent motion(IVIM)diffusion weighted imaging(DWI)parameters ofwith different T and M stage prostate cancers.Methods Data of 46 patients with prostate cancer who underwent routine MR imaging and reduced field-of-view IVIM-DWI were retrospectively analyzed.The standard apparent diffusion coefficient(ADC_(standard)),diffusion coefficient(D),pseudo diffusion coefficient(D)and rapid diffusion score(f)were compared among prostate cancers with different clinical T and M stages and Gleason scores,and the correlation of T stage,M stage,Gleason score and the above parameters were explored.Results There were significant differences of ADC_(standard),D and f among prostate cancers with different T stages(all P<0.05).ADC_(standard)and f were significant different between T2 and T3,T2 and T4 stage,also of D between T2 and T4,T3 and T4 stage prostate cancers(all P<0.05).There were significant differences of ADC_(standard)and D among prostate cancers with different M stages and Gleason scores(all P<0.05).T stage of prostate cancer was negatively correlated with ADC_(standard),D and f(r=-0.69,-0.75,-0.53,all P<0.05),while M stage and Gleason score were both negatively correlated with ADC_(standard)(r=-0.34,-0.50,both P<0.05)and D(r=-0.43,-0.53,both P<0.05).Conclusion Significant differences of ADC_(standard),D and f existed among prostate cancers with different T stages,which were all negatively correlated with T stage.There were significant differenced of ADC_(standard)and D between prostate cancers with different M stages,which were negatively correlated with M stages.
