心理健康教育为主的综合干预对患子宫肌瘤需行经腹子宫全切术年轻患者的心理应激反应的作用研究 被引量:2

摘要 目的研究心理健康教育为主的综合干预运用于子宫肌瘤需行经腹子宫全切术年轻患者中的价值。方法选择南阳市中心医院2020年2月至2021年6月接收的80例子宫肌瘤患者,以随机排列法进行分组,分为研究组和对照组,每组40例。研究组开展心理健康教育为主的综合干预,对照组实施常规护理,统计两组失血量、手术时长、下床时间、排气时间、胃肠恢复时间及住院天数。统计护理前后测评两组抑郁、焦虑自评量表(SDS、SAS)。统计护理前后依从性、认知能力及自我效能感(GSES评分)。统计两组并发症(皮下气肿、腹胀、尿痛)情况,比较两组护理结果。结果研究组失血量、手术时长、下床时间、排气时间、胃肠恢复时间、住院天数均少于对照组,差异有统计学意义(P<0.05)。护理前两组SAS、SDS差异无统计学意义(P>0.05),护理后研究组评分均低于对照组,差异有统计学意义(P<0.05)。研究组并发症发生率(7.50%)低于对照组(27.50%),差异有统计学意义(P<0.05)。护理前两组依从性、认知能力差异无统计学意义(P>0.05),护理后研究组评分均高出对照组,差异有统计学意义(P<0.05)。两组干预前GSES评分差异无统计学意义(P>0.05),干预后GSES评分明显提高,与对照组相比,观察组评分明显更高,差异有统计学意义(P<0.05)。结论心理健康教育为主的综合干预具有更佳疗效,可促进病情尽快稳定,减少负性情绪,提升患者自我效能感,患者依从性及认知能力明显提升,同时减少并发症,安全性高。 Objective To study the value of comprehensive intervention based on mental health education on young patients with hysteromyoma undergoing total hysterectomy via abdomen.Methods A total of 80 patients with hysteromyoma received from Nanyang Central Hospital from February 2020 to June 2021 were randomly divided into study group and control group with 40 patients in each group according to the method of random arrangement.The study group carried out comprehensive intervention focusing on mental health education,while the control group carried out routine nursing,and the blood loss,operation duration,time out of bed,exhaust time,gastrointestinal recovery time and hospitalization days of the two groups were counted.The self-rating scale of depression and anxiety(SDS,SAS)of the two groups were evaluated before and after nursing.Nursing compliance,cognitive ability,general self-efficacy scale(GSES score)and the complications(subcutaneous emphysema,abdominal distension and urination pain)in the two groups were statistically analyzed,the nursing results were compared.Results The blood loss,operation time,time to get out of bed,exhaust time,gastrointestinal recovery time and hospitalization days in the study group were less than those in the control group,and the difference was statistically significant(P<0.05).There was no significant difference in SAS and SDS between the two groups before nursing(P>0.05),but the scores in the study group were lower than those in the control group after nursing(P<0.05).The incidence of complications in the study group(7.50%)was lower than that in the control group(27.50%),with a statistically significant difference(P<0.05).There was no significant difference in compliance and cognitive ability between the two groups before nursing(P>0.05),but the scores in the study group were higher than those in the control group after nursing(P<0.05).There was no significant difference in GSES scores between the two groups before intervention(P>0.05),but after intervention,GSES scores were significantly improved,compared with the control group,the scores in the observation group were significantly higher,with a statistically significant difference(P<0.05).Conclusion The comprehensive intervention based on mental health education has better effect,which can promote the stability of the disease as soon as possible,reduce negative emotions,enhance the self-efficacy of patients,improve the compliance and cognitive ability of patients,and reduce complications,with high safety.
