心房颤动伴慢性心力衰竭患者射频消融术后行运动康复的疗效 被引量:7

摘要 目的观察心房颤动(AF)伴慢性心力衰竭(CHF)患者行射频消融术转为窦性心律后,12 w运动康复对CHF的疗效。方法纳入确诊的AF伴CHF行射频消融术成功后患者50例,随机分为对照组25例和观察组25例。对照组进行常规药物治疗CHF,观察组在药物治疗基础上加上运动康复治疗12 w。记录和分析两组运动康复前后氨基末端前体脑钠肽(NTpro-BNP)、左房舒张末期直径(LAEDD)、左室舒张末期直径(LVEDD)、左心室射血分数(LVEF)、摄氧峰值(peak VO_(2))、无氧呼吸阈值(AT)、6 min步行距离(6MWD)、明尼苏达心力衰竭生活质量调查表评分(MLHFQS)及再住院率。结果运动康复前,两组NTpro-BNP、LVEF、LAEDD、LVEDD、peak VO_(2)、AT、6MWD及MLHFQS差异均无统计学意义(P>0.05)。12 w运动康复后,观察组NTpro-BNP水平、MLHFQS评分较运动康复前显著降低(P<0.05),peak VO_(2)、AT、6MWD较运动康复前显著增加(P<0.05);对照组以上指标差异均无统计学意义(P>0.05)。12 w运动康复后,与对照组相比,观察组NTpro-BNP水平、MLHFQS评分显著降低(P<0.05),peak VO_(2)、AT、6MWD显著增加(P<0.05);两组再住院率相比差异无统计学意义(P>0.05)。结论AF伴CHF患者行射频消融术转为窦性心律后,12 w运动康复可以显著改善机体功能和生活质量,对心脏结构及功能改善作用有限。
