Vital Stim电刺激配合Mendelssohn吞咽法治疗脑卒中患者咽期吞咽障碍的疗效观察

Efficacy of Vital Stim electrical stimulation combined with Mendelssohn’s swallowing method in the treatment of pharyngeal dysphagia in stroke patients

目的:观察Vital Stim电刺激配合门德尔松(Mendelssohn)吞咽法治疗脑卒中患者咽期吞咽障碍的临床疗效。方法:选取我院72例脑卒中合并咽期吞咽障碍的患者,随机分为对照组和观察组,每组36例,对照组采用吞咽障碍的常规治疗和Vital Stim电刺激,观察组在对照组的基础上配合门德尔松(Mendelssohn)吞咽法,2组均连续治疗3周,观察两组治疗前后洼田饮水试验分级情况,标准吞咽功能评估量表(SSA)评分情况。结果:治疗前各项评估等级在两组之间无显著差异(P>0.05),治疗后观察组在洼田饮水试验评估中的总有效率为88.89%,优于对照组总有效率(69.44%),两组差异有统计学意义(P<0.05);两组患者的洼田饮水试验和SSA评估等级均有所下降,且观察组下降优势较对照组有显著性(P<0.05)。结论:Vital Stim电刺激同时配合门德尔松(Mendelssohn)吞咽法治疗脑卒中患者咽期吞咽障碍的疗效更佳。

Objective:To observe the clinical efficacy of Vital Stim electrical stimulation combined with Mendelssohn’s swallowing method in the treatment of pharyngeal dysphagia in stroke patients.Methods:72 patients with cerebral apoplexy complicated with pharyngeal dysphagia in our hospital were selected and randomly divided into 2 groups.The control group received routine treatment of dysphagia and Vital Stim electrical stimulation,while the observation group received Mendelssohn swallowing method on the basis of the control group.Both groups received continuous treatment for 3 weeks,then,to observe the grading of the water swallow test and the score of the Standard Swallowing Assessment in the two groups before and after the treatment.Results:The evaluation level between the two groups before treatment had no significant difference(P>0.05),after treatment,the total effective rate in the observation group of the grading of the water swallow test was 88.89%,better than that of the control group(69.44%),the difference between the two groups was statistically significant(P<0.05).The evaluation grade of SSA and drinking water test decreased in 2 groups,and the descending advantage of observation group was significantly higher than that of the control group(P<0.05).Conclusion:Vital Stim electrical stimulation combined with Mendelssohn swallowing method is more effective in the treatment of pharyngeal dysphagia in stroke patients.
