老年患者急性缺血性脑卒中合并尿路感染特征及相关因素分析

Characteristics and related factors of urinary tract infection in elderly patients with acute ischemic stroke

目的对老年急性缺血性卒中(AIS)患者合并尿路感染特征及危险因素进行分析,为临床预防及治疗提供参考。方法回顾性分析2020至2022年医院收治的332例老年AIS患者合并尿路感染的特征及危险因素,并使用受试者工作特征(ROC)曲线对回归模型进行评价。结果332例AIS患者中发生尿路感染81例,感染率为24.4%。G-杆菌中的大肠埃希菌与G+球菌中的肠球菌为主要致病菌。感染组与对照组患者年龄、性别、是否合并糖尿病、是否留置尿管、格拉斯哥昏迷评分量表(GCS)、住院天数比较,差异有统计学意义(P<0.05)。两组回归分析结果显示,年龄≥80岁、女性、合并糖尿病、留置尿管、格拉斯哥昏迷评分量表(GCS)评分降低、住院天数≥10天为AIS患者合并尿路感染的独立危险因素(P<0.05)。结论年龄≥80岁、女性、合并糖尿病、留置尿管、GCS评分降低、住院天数≥10天为AIS患者合并尿路感染的独立危险因素。临床可根据感染特征与相关因素,有针对性进行干预。

【Objective】Characteristics and risk factors of urinary tract infection in elderly patients with acute ischemic stroke(AIS)were analyzed to provide reference for clinical prevention and treatment.【Methods】The characteristics and risk factors of urinary tract infection in 332 elderly AIS patients admitted to the hospital from 2020 to 2022 were reviewed.The ROC curve was used to evaluate the regression model.【Results】Urinary tract infection occurred in 81 of 332 AIS patients,with an infection rate of 24.4%.Escherichia coli in G-bacillus and enterococcus in G+coccus were the main pathogenic bacteria.There were statistically significant differences between the infection group and the control group in terms of age,sex,diabetes,urinary catheter retention,GCS score,and hospitalization days(P<0.05).Regression analysis showed that,age≥80 years,female,diabetes mellitus,indwelling urinary catheter,decreased GCS score,hospitalization days≥10 days were independent risk factors for AIS patients with urinary tract infection(P<0.05).【Conclusion】Age≥80 years,female,diabetes mellitus,indwelling urinary catheter,reduced GCS score,length of stay≥10 days were independent risk factors for urinary tract infection in AIS patients.Clinical intervention can be carried out according to the characteristics of infection and related factors.
